Story of Practo :

The founders Abhinav Lal and Shashank ND launched in the year 2010.

Shashank ND was frantically getting together his father’s medical record and scanning them.

He wanted to get a second from the American physician, for an operation that his father was about to undertake.

If that does not bed enough, he was also unable to access enough information about the doctor.

To make an informed decision before putting his father’s life in his doctor’s hands.

This incident urged Shashank and his classmate, Abhinav Lal, to set up Practo Technologies Pvt. Ltd.

The Duo founded the company in Bengaluru, in 2008, when they were still college students in their final year of engineering (B. Tech, computer engineering) at National Institute of Technology, Karnataka.

Though numerous players have come into the practice management space since began, Shashank believes that razor focus on product quality and support has enabled it to maintain a steady lead.

The only pervasive problem would be the inability of some of the old school doctors and patients to convert as they would be too well adapted to the prevalent system and would not be changed.

The reputation of these doctors over the years would give them a cushion and would not necessitate them to adapt to changing technologies.

Challenge faced:

Targeting these doctors or clinics would be a challenging task but they would have a large enough market even if it does not focus on the members of the old school healthcare system.

Practo – India’s home for health

practo app case study

A technology platform to help billions lead healthier lives

Practo is a health tech company based out of India and aims to connect the fragmented medical community with millions of patients across the country. Practo has made information more accessible by creating a platform that allows doctors to list their services and manage patient interactions and provides an array of services for patients, mainly for booking interactions with doctors and maintaining a consistent medical history.

Practo targeted independent healthcare professionals across cities and small towns who were often oversubscribed and lacked sophisticated resources. It created a SaaS product for doctors to easily organize appointments, track past medical records and provide e-prescriptions. The platform enabled doctors to save valuable time, establish their presence or grow their business and engage more meaningfully with patients than ever before. While independent doctors and small clinics were the main target of its product, Practo created a multitude of products and services for consumers to enjoy network effects on this platform. Practo reduces the search and transaction costs for patients (consumers) and helps users find the right doctors through easily accessible and contextually relevant listings. As a valuable extension, Practo provides health-centric credit cards and allows patients to order medicines online.

practo app case study

Practo has smartly captured value through its platform and deeply subsidized the consumers part of the network. Its revenue model centers around the medical providers (clinics) and Practo makes most its money through the SaaS subscription software. For patients, the listings are free, and the company doesn’t charge any commissions. By increasing efficiency and visibility of small clinics, Practo wants to make the platform sticky on the supply side. Similarly, by making the demand side free, it aims to retain patients on account of its superior product the ability to maintain health history (especially in the absence of a national system) and an ever-growing array of medical services and adjacencies.

Practo has deployed a feet-on-ground sales strategy, which is expensive but effective. Due to lack of any organized system, the platform is very sticky for doctors and will make the economics work – a long time commitment from doctors has ensured that the lifetime value (LTV) of its product is very high and justifies the higher customer acquisition cost (CAC) initially. Finally, data captured by Practo (access to over 10+ M medical records) is a huge value capture option which is yet to be exercised. No other system in the country has the scale nor richness of data compared to Practo and it opens a vast pool of businesses that Practo can venture into.

Pathways to a Just Digital Future

Practo’s model has already proved to be scalable with their market entries into multiple other countries like Malaysia, Brazil, Singapore. Their strategy to scale is simple – onboard as many doctors and clinics onto the platform and drive value for them. Then, by heavily subsidizing the consumers and providing good services, patients would follow. Their on-foot sales approach seems cumbersome, especially in different markets but the high LTV justifies the CAC that Practo may occur in different countries.

Practo has set itself up in a way to avoid disintermediation. The governing environmental factors that keep patients/consumers locked-in include the vast array of health-related services available on Practo’s platform and the lack of a viable alternative. Similarly, doctors are benefiting from the scale and efficiency the software provides and are more incentivized to take the patients through the system rather than bypass it. Since it is a commission-free model, doctors are not worried about Practo taking a cut and prefer using the systems to save time. Multi-homing threat is also reduced since the company is storing all health-records in one location for the users’ convenience, avoiding the temptation to move to other solutions on different occasions.

practo app case study

In most cities across the country, Practo has been a first mover connecting local communities with healthcare services. This has created small clusters where Practo is operational and also opens them up to local competition. This business has smaller geographically concentrated clusters like Uber in terms of network effects. In specific areas where Practo has not expanded, local competitors can come up with similar products and lock-in that community of doctors and potential patients onto their platform. Thus, scaling quickly and nipping competition in these smaller clusters must be Practo’s focus.

In the future, the platform will continue to enjoy and thrive on network effects. The reviews and ratings on Practo will ensure same-side network effects as people want to work with trusted sources and learn about their peer’s experience. Similarly, the two-sided network effect will be enhanced when more users on the platform will prompt and incentivize more clinics to reach that audience, thereby creating a virtuous cycle.

A strong platform, focus on simplifying the healthcare process and creating value-add services for both doctors and patients makes Practo poised to emerge as a massive winner in the Indian health tech industry.

Student comments on Practo – India’s home for health

Interesting article. I wonder about the ethics of “locking” clinics and doctors to a single platform. What if a customer uses a competitors system? Will it interfere with their ability to get treated? I see a risk of multi-homing here – whichever system a medical provider is using is the system the patient will engage with, up to as many platform as they need to engage with. And since the costs are low for the customer, it doesn’t cost them anything to be on multiple platforms. But then comes the question of how medical information is transferred, since I assume it’s not owned by the customer. I agree though that Practo may have the biggest competitive advantage in simple being the first mover in cities and countries. By not giving customers the opportunity to even know about its competitors, I think Practo could maintain it’s leader position.

Thank you for sharing about this service! I hadn’t heard of it before. I know in India there are so many private healthcare providers and most consumers do not have health insurance, so there is a much greater need for direct to consumer advertising (which is everywhere on the streets). I also have heard many medical practices communicate with their patients over WhatsApp today. I think that is the biggest competitor for them and why they can’t afford to charge commission. Time will tell whether they really provide enough value to their users over such an accessible alternative or if they might have to expand into other value-added services in this space such as owning their own doctors and being on the platform themselves, or providing quality control measures.

The caselet on Practo elucidates the business model of a matching platform, one that matches the users on one side (clinics) with those on the other side (patients). The case elaborates on the business model, and elucidates why a combination of the SaaS offering (Practo Ray) and the platform business model (Practo.com) is essential to the sustainability of the business model.

This case is an extension of Chapter 7 Network Mobilization.

R Srinivasan, Professor of Strategy, prepared this case for class discussion. This case is not intended to serve as an endorsement, source of primary data, or to show effective or inefficient handling of decision or business processes.

Practo Case Answers

1. introduction – practo.

Practo, an online-based health service provider, was founded by Shashank N. D. and Abhinav Lal out of frustration with the inefficiency and disorganization of the healthcare facilities in India. The company’s HQ is situated in Bangalore, the center of India’s high-tech industry.

Since its foundation in 2008, the company has expanded and works with more than 200,000 clinics across 15 countries (Chauhan & Kumar, 2013). Both founders have engineering backgrounds and are involved in product development and marketing processes. Along with the two founders, a team of 200 employees.

The first part of this paper will look at India’s business environment to understand the factors that contributed to Practo’s foundation and growth. The second part will look at the company’s strategy and management practices leading to its success. Lastly, this paper will discuss potential growth opportunities in Asia and in the United States.

2. India’s Business Environment

2.1. history of india.

India is the seventh-largest country in Southeast Asia by area and is the second-most densely populated country. Also, it is one of the most populous democracies in the world after it achieved independence in 1947. Prior to independence, India was under British rule.

Being in Southeast Asia, India is well connected by both land and sea to its neighboring countries. By land, China, Nepal, and Bhutan are on the northwest, and Bangladesh and Myanmar are on the east. Thailand and Indonesia share the maritime border with India. The British took advantage of India’s strategic position in carrying out their trade activities, but the growth was stagnant, and India ultimately lost its share in global trade.

2.2. Political dimension

Having been ruled under the British for many decades, there was a growing urge for nationalism in India, which was headed by Mahatma Gandhi in 1920. After successfully gaining independence in 1947, India then had its own constitution created in 1949. India is a parliamentary democratic republic, whereby the president is the head of state, and the prime minister oversees running the federal government.

Amid gaining independence, Gandhi advocated to the Indian population that they had to be united, regardless of religion, caste, and creed, in their fight for independence. This was, however, not followed and resulted in many riots amongst the religions. In 1946, there was also a major riot that resulted in the most bloodshed.

The political system in India can be categorized into three main types, namely, national parties, state-recognized parties, and registered unrecognized parties. The parties that run the government are the national parties, of which the Bharatiya Janata Party, also known as the BJP, and the Indian National Congress dominate the political scene in India (Pariona, 2017). Both parties share the common ideology for the economy to be liberalized and have also incorporated Gandhi’s idea of being one common nation.

With the political party having such an ideology, it gives rise to more economic opportunities for the country. Furthermore, a liberalized country is more attractive to foreign investors, which will then bring in more investment for the country. This further boosts economic growth in terms of international trade and foreign direct investment. Some of the country’s key trading partners include China and the USA.

2.3. Economical dimension

As much as India is becoming a popular country for foreign investors, its local economy is also fast growing due to its people being one of their key resources (Bhargava, 2016). The service industry of India contributes the largest percentage of 59% to the annual GDP growth.

With the service industry being the biggest in India, it is an added incentive for foreign businesses to shift labor-intensive jobs to India. For example, call centers are common in India whereby large multinationals such as Singapore Airlines and Telstra set up their call centers in India to have a lowered labor cost.

Apart from the service industry, the informational technology sector is also a fast-growing sector in the Indian economy, with a 5.9% contribution to the GDP and an annual growth rate of about 22%.

Both the service and technological factors create a strong base for start-up companies as these are two resources that are key contributing factors to the development of businesses. India also has many home-grown global players, such as TATA and ONGC. The combination of urbanization and the people of India are the key driving forces of economic growth.

2.4. Cultural & Social dimension

While importance is placed on India’s large population as a driving economic force, it is also notable that 65% of this population consists of the young below the age of 35 (Government of India). There are five main socio-cultural factors that affect entrepreneurs in India. These factors are family support, education, caste, social network, and religion.

The caste system is one of the stronger factors as a social hierarchy is set from birth, and this stands to be a great disadvantage to those born into a lower caste. Individuals born into an upper class have more access to financial and non-financial resources, whereas those born in a lower class are simply banned from entrepreneurial activity.

The education system in India has been improving over the years, with an increasing number of colleges and universities being set up. This goes to show that more individuals are gaining an education and is reflected based on the literacy rate, which is currently 74.04%, which is deemed high for a developing country (Government of India).

2.5. Influence on Practo

Practo has been able to achieve success in India mainly due to the fast-growing IT industry and the demographics of the population. With its main product being software, it is to the company’s advantage that the large population is made of young individuals who are more receptive to IT products.

Furthermore, with the prevalence of better and higher education, these individuals are better equipped with the skillset to use such technologies.

A cultural trend that has also contributed to the company’s success is the fact that the trust in Indian government hospitals is less compared to private. Practo targets two main user segments: private doctors and the general public in need of medical help.

As India is a developing country, there is great room for improvement with regard to improving the efficiency of the healthcare system. Coupled with the trend of increased trust for private doctors, Practo was desirable to both doctors and customers.

Doctors were able to carry out more effective business operations for saving patient records, and users were also able to seek the best medical advice needed. Therefore, these two factors have been the driving force for Practo’s success.

3. Strategies

3.1. overall strategy.

The strategy of the founders of Practo was to use…

  • Practo needed a scalable solution to cut costs, improve efficiency and accelerate turnaround time.
  • MSB identified an opportunity as how doctors can identify themselves and sign their insurance agreements faster.
  • MSB Docs streamlined the signing process at Practo and reduced the processing cycle time from days to just hours.
  • Introduction

Business Challenge

The solution, the company.

Founded in 2009, Practo is a leading integrated healthcare company that brings seekers and providers of medical expertise on a common platform. Right from finding and booking an appointment with verified doctors, booking diagnostic tests, obtaining medicines, to storing health records, etc, Practo provides patients with a wide range of healthcare services that are high-quality, trusted and affordable. The digital healthcare expert has been leveraging the power of new-age technologies to impart superior experiences to the masses.

Practo has a large, diverse team comprising of over 2,500 employees. The company keeps increasing its pool of people in order to fulfil the evolving business requirements. Human Resources (HR) therefore plays a pivotal role in the organisation, since new employees need to be continually recruited, onboarded and engaged within the organisation. Besides this, Practo also continues to tie up with more and more doctors globally on an ongoing basis. All this involves a great deal of signing and documentation work.

Traditionally, prior to integration with MSB Docs, all HR processes including employee onboarding, engagement, payroll, employee exits, etc were conducted manually at Practo. Important documents such as job contracts, background check reports, contract extensions letters, offer letters, NDAs or confidentiality agreements, etc had to be manually generated and circulated around to gather physical, ink signatures. Often, signatories were located across different company offices, in the case of which print outs were taken and shipped out to different locations. The signed documents were then collected back and manually archived. The same process was repeated for tying up with doctors globally.

Over the period of time, the company realized that the manual way of signing and documentation was extremely slow, exhausting and inefficient. A few of the challenges that Practo faced are listed below.

  • Delayed Employee Onboarding The manual way of document handling resulted in a high document turnaround time, due to which it often took several weeks to finalize and onboard a new hire. With employees joining later than expected, the output at work gradually started getting impacted. Also, such sharp delays many a times led to a loss of interest from the candidates, which could also hamper the company’s reputation.
  • Decreased HR Productivity The HR team at Practo was spending too many hours in conducting recurring paperwork. This involved taking document printouts, running around to get hard copies filled and signed, faxing/ couriering documents, storing them in physical cabinets and locating them for audit purposes. These routine, administrative tasks took a toll on the productivity of HR personnel, who were unable to perform other tasks such as employee engagement, retention, etc.
  • Greater Chances of Errors The pen and paper way of filling and signing documents invited greater chances of errors. Often, there were instances of forms being incompletely or incorrectly filled. These forms had to then go back and forth multiple times before finally getting approved.
  • Storage Issues Since the paper-based records were physically stored, retrieval of information was quite a difficult task. Also, physical storage of documents led to greater security issues, which could possibly invite troubles from the compliance, audit and regulatory perspective.
  • High Costs The cost of paper and paper-based processes was significant for the company and was severely impacting their revenues.

As Practo continued to grow, it felt an acute need to streamline its complex signing/ documentation processes, enhance efficiency and minimize costs. The health-tech platform wanted to do away with the existing operational inefficiencies/ bottlenecks and was thus looking for a new-age, scalable solution for the same.

MSB Docs enables businesses of all kinds to adapt seamless, error-free workflows.

Practo turned to MSB Docs for incorporating its advanced electronic signatures into their workflows. MSB Docs took the lead and automated all the HR workflows of the company. The highlights of the eSignature solution implemented by MSB Docs are given below

  • All the physical HR documents were seamlessly converted into electronic documents, and physical signatures were replaced with electronic signatures.
  • HR professionals at Practo can now simply send a link to the applicants, clicking on which they can review, complete and sign the onboarding documents directly from any internet-enable device. As a result, all forms can now be signed and completed speedily within a few minutes, eliminating delays which were present in the manual onboarding process.
  • Once the forms and documents are electronically signed, they get stored automatically, and one can quickly move on to the next stage of employee onboarding.
  • The HR professionals at Practo can now get multiple employee contracts, forms and other documents simultaneously and speedily.

At Practo, paper-based documentation and manual signatures have become a thing of the past. The company is now able to operate in a time and cost-efficient manner, with the following benefits being reaped

  • Improvement in document turnaround time – As the cumbersome processes of printing, scanning, faxing, couriering, sending attachments, etc were eliminated, Practo was successfully able to reduce its document processing time by a whopping 79%.
  • Reduction in costs – As all contracts and documents are now created and signed digitally, the company has been able to save significant costs related to paper and paper-based processes.
  • Enhanced HR Productivity – As the HR employees now spend lesser time on administrative paperwork, they are able to productively perform other valuable tasks such as scouting for new talent, employee engagement, training, retention, etc.
  • Secure storage leading to improved compliance – For Practo, ensuring safety of patient records and other critical information is extremely important. With MSB Docs on board, all the company’s documents are now securely stored and safeguarded against fraud, tampering or any external threat. This has in turn ed to improved compliance.
