The journey to parenthood

Mother kissing baby

Congratulations! Whether you are pregnant or just beginning to plan for a baby, MedStar Health would like to be your resource for information. Since it's our business to know first-hand the many decisions you face, we want to help you with the planning stages of your pregnancy.

Preconception

If you are planning to become pregnant:

Make an appointment to see your physician

Review your personal medical history and note any missed vaccinations, childhood diseases

Check your diet—eat plenty of fruits and vegetables

Abstain from any alcohol, drugs, or smoking for the duration of your pregnancy

The first trimester

Make your first prenatal appointment  as soon as you think you are pregnant

Check with your health plan and doctor for information on parent education classes

Your health care provider may recommend a vitamin supplement

Morning sickness may set in with your first missed period and persist through the first trimester—bland carbohydrates, like bread and crackers, may help nausea

Expect the onset of cravings and mood swings: your body is readjusting its hormone levels

Expect sensitivity to strong odors and gases

  • Your baby experiences everything you do
  • Strong chemicals, cleaning solutions, and even natural gas can be hazardous to your baby's development

You will notice your clothes are getting tighter

Mile markers

4th week: Your baby is approximately a quarter-inch long. The cells that make up the heart begin to beat. Eyes and ears are visible.

8th week: Your baby is now one inch long, with noticeable head growth (signifying brain development), facial features, and more defined appendages.

12th week: Your baby is approximately three inches long and weighs one ounce. Vital systems are forming, and it may be possible to determine the baby's gender.

The second trimester

Good news—if morning sickness has not ended, it will soon. In no time you will feel the best you have in months

Investigate classes to help you reduce stress, increase comfort, and prepare yourself and family members for birth

Just as important as eating well throughout your pregnancy, drink plenty of water. Water relieves constipation, and a healthy diet helps your baby increase its body fat during this stage.

16th week: Your baby is four to  six inches long and weighs up to four ounces. You will feel the baby move more, and a heartbeat can be detected with special equipment.

20th week: Your baby is approximately 10 inches long and weighs up to one pound. The baby may feel more active, and a clear heartbeat can be heard.

24th week: Your baby is 12 to 14 inches long and weighs up to 24 ounces.

The third trimester

Prepared childbirth classes are a must by the seventh month. You due date is only an approximation, and you want to be prepared. Call the Hospital Center's Parent Services department at 202-877-6161 to schedule classes.

Make decisions on circumcision

Make birthing plans. The delivery day can come at any time, and you should have a game plan for the big event.

Look for infant care and support. If you will be returning to work you will want to supervise any caretaker or facility for the first couple of days to ensure everything goes well. If you plan to breastfeed, check with your employer's Human Resources department for a place to pump.

By the eighth month, install a car seat. Follow the manufacturer's instructions closely and be sure to keep the car seat out of the front seat and away from all airbags.

As you approach delivery, your baby will move into position in preparation for birth. Lightening, when the baby's head descends into the pelvis, can happen at any time and is not necessarily an indication that labor is about to begin.

28th week: During the seventh month, the infant is approximately 15 inches long and up to two and a half pounds. Your baby's eyelids may be open, and the baby can distinguish light and sound outside the uterus.

32nd week: The infant is nearly 16 inches long and weighs about four pounds. The baby will begin sleeping and waking.

36th week: During the ninth and final month, the infant is approximately 19 inches long and weighs seven to eight pounds. The baby will continue to gain approximately one half pound a week.

Midwife-led birth centers can help fill gaps from maternity ward closures

Photo of a hospital building with a sign in front saying that it is closed.

February 27, 2024 — In the U.S., more than two million women of childbearing age live in so-called “maternity care deserts,” defined as counties lacking obstetric care. Most maternity care desserts are in rural areas—in fact, 55% of rural hospitals offer no maternity care.

The reasons cited for hospital maternity ward closures include declining birthrates, staffing shortages, and increased costs. But research by Alecia McGregor , assistant professor of health policy and politics, has found other common threads—hospitals that appear most likely to close are often those most reliant on Medicaid, which reimburses at lower rates than commercial insurers; and obstetric unit closures, specifically, are more common in hospitals that serve a larger share of Black patients.

McGregor has suggested that midwife-led birth centers could be part of the solution to filling gaps in care caused by maternity ward closures. She is quoted in a February 26 Milwaukee Independent article: “The kinds of lifesaving procedures that can only be conducted in a hospital are important for those very high-risk cases. But for the majority of pregnancies, which are low-risk, birth centers can be a very important solution to lowering costs within the U.S. health care system and improving outcomes.”

U.S. News Announces Inaugural Edition of the Best Hospitals for Maternity

USNWR Inaugural Edition of Maternity

Washington, D.C. – U.S. News & World Report today published the inaugural edition of the Best Hospitals for Maternity , which expands the U.S. News Best Hospitals rankings, ratings and data offerings. This expansion builds upon more than 32 years of guiding patients and families to the best health care decisions at every stage of life.

“All families deserve to be informed on how hospitals perform on key indicators of quality, which is why U.S. News has compiled and published a trove of maternal health data from hospitals across the country,” said Ben Harder, managing editor and chief of health analysis at U.S. News & World Report. “Now, hundreds of hospital profiles on USNews.com feature relevant, targeted information for parents-to-be to use during their search to find – in consultation with their healthcare provider – the best local hospital for an uncomplicated pregnancy.”

The Best Hospitals for Maternity evaluation looks at hospital data relating to only uncomplicated pregnancies – not high-risk pregnancies – and at five different factors: scheduled early deliveries, C-section rates in low-risk women, newborn complications, rate of exclusive breast milk feeding and option for vaginal births after cesarean. Unique to U.S. News, participating hospital profiles also include a variety of relevant information on services and amenities, like private rooms, valet parking and availability of child birthing classes.

“Hospitals that performed well had fewer newborn complications, fewer early deliveries and fewer C-sections compared to other hospitals across the nation. Out of 2,700 hospitals nationwide that offer maternity services, we identified 237 that earned recognition among the Best Hospitals for Maternity,” said Harder.

In order to be recognized among the Best Hospitals for Maternity, a hospital had to submit data from 2019 and its performance had to meet U.S. News’ maternity care standard. Data submission was voluntary; there was no fee to participate in the evaluation. For more information about the Best Hospitals for Maternity, please refer to the methodology .

Editorial pieces accompanying the evaluation include pieces on what to know about VBAC , questions to ask when choosing a maternity hospital , and benefits of breastfeeding .

  • Maternity Care (Uncomplicated Pregnancy)

Overall Rating

Scheduled early deliveries.

Never or rarely schedules delivery earlier than recommended.

To protect the health of the baby, experts recommend against scheduling induction or C-section prior to the 39th week of pregnancy, unless there is an important medical reason for early delivery.

Excellent at minimizing avoidable C-sections.

Less than 23.9 % of first-time, low-risk pregnancies at full term were delivered by Cesarean section. While some C-sections are necessary operations, unnecessary C-sections should be avoided because they pose long-term health risks to baby and parent.

Newborn Complications

Newborn complications are rare .

In full-term newborns with normal birthweight and no preexisting conditions, complications such as infection or injury are rare. Some hospitals have even fewer complications than others. The average rate at hospitals was 3.2%. At this hospital, 2% to 2.6% of lower-risk births involved newborn complications.

Breast Milk Feeding

73.7 % of babies were exclusively breastfed or fed breast milk during their hospital stay.

The average in the West is 57.6 % and the national average is 49%. Feeding breast milk boosts an infant's immune system and brain development and may protect against allergies, asthma, obesity, and other conditions.

Vaginal Birth After Cesarean (VBAC)

Routinely offers to support VBAC with the rate of 33.7 %.

A pregnant patient who previously has had a C-section may prefer to attempt VBAC rather than undergo another C-section. Some hospitals can accommodate this preference.

This hospital's episiotomy rate is less than 5% .

Episiotomy, a cut made in the vagina during childbirth, rarely should be performed. Routine use is not recommended by experts except for specific clinical circumstances.

Birthing-Friendly

This hospital met criteria to be recognized as Birthing-Friendly because it participated in a quality improvement collaborative and implemented patient safety practices in order to improve maternal outcomes.

Hospitals recognized as Birthing-Friendly have demonstrated their commitment to improving maternal outcomes by participating in quality improvement collaboratives and have implemented one or more projects related to maternal morbidity to address complications.

Transparency on Racial/Ethnic Disparities

This hospital tracked and reported its race- and ethnicity-specific performance on all of several performance measures for which racial and ethnic disparities may exist.

Whether this hospital tracked and reported its race- and ethnicity-specific performance of measures for which racial and ethnic disparities may exist. Transparency is important to patient choice of care and in raising industry standards.

Breaking News

Health | disgraced pharmacy worker charged with murder in south bay overdose, health | santa clara county sends letter to regional hospital warning closure of maternity ward may violate agreement, the nurses union also sued the hospital over closing labor and delivery services.

SAN JOSE, CALIFORNIA - APRIL 23: Aileen Tran, an ICU registered nurse, with her son Caleb, 15-months, outside of Regional Medical Center in San Jose, Calif., on Thursday, April 23, 2020. They were protesting the planned closure of Women's Services. (Nhat V. Meyer/Bay Area News Group)

Santa Clara County has sent a letter to Regional Medical Center of San Jose warning that the closure at the end of May of its maternity ward may jeopardize the hospital’s current Level II trauma designation.

The move comes days after the California Nurses Association, the union that represents nurses at the hospital, sued Regional , which is part of HCA Healthcare, in a bid to prevent the closure of the labor and delivery wing. The suit argued it could leave pregnant women in East San Jose arriving through the emergency room in the care of nurses and doctors without adequate labor and delivery expertise. The request for an injunction to prevent the closure was denied.

Regional spokeswoman Sarah Sherwood said in a statement, “Regional Medical Center is working hard to ensure that the closure of its OB department goes smoothly for its remaining OB patients. We stand by our reasoning to close based on a lack of demand due to a falling birth rate in the Bay Area. We are committed to delivering excellent care in many different critical areas of medicine and will ensure a smooth transition for all OB patients. We are not surprised to learn that the formal opposition by CNA to the closure was officially denied by the District Court on Friday. We will continue to provide emergent OB care through our emergency department; in line with our robust trauma center.”

In the May 29 letter to Regional CEO Tomi Ryba, county counsel James Williams and deputy county counsel Wesley Dodd said that the hospital’s agreement with the county requires the hospital to provide obstetric and gynecological services staffed by qualified specialists and services to any adult major trauma victims.

“It is not clear how RMC will maintain compliance with the above provisions of Annex D if its labor and delivery department is closed,” the pair wrote, noting that labor and delivery nurses were laid off as part of the closure. “Further, it appears RMC may be unable to provide medically necessary emergency services, including but not limited to ongoing fetal heartbeat monitoring, to pregnant trauma patients who present in RMC’s emergency department, and RMC would be forced to transfer those patients to a trauma center that is equipped to provide such services.”

If the hospital doesn’t respond by then, the letter says, the county may “impose a plan of correction” or end its agreement with Regional. As of Thursday afternoon, the county said it had not received a response.

In a statement, the union called on the hospital to reopen the closed unit.

“To protect East San Jose women and families who have long counted on this hospital for birthing care,” said California Nurses Association President Malinda Markowitz, “HCA should demonstrate its commitment to the community by restoring full maternal-child care, at Regional.”

Several Chicago suburbs likely are losing more than 40% of water. And some Georgia systems are losing more than 80% of their treated drinking water, said Sunil Sinha, a water researcher at Virginia Tech.
