Keynote speech to the world health summit 2021 – 24 october 2021, unicef executive director henrietta fore.

Excellencies, colleagues, friends … it is a pleasure to be with you here today for the World Health Summit.  

I am honoured and inspired by the spirit of collaboration among experts in science, politics, business, government and civil society represented at this Summit.   

On behalf of UNICEF, I am grateful for the opportunity to speak with you now at this critical moment in the global response to the COVID-19 pandemic – a pandemic which continues to impact so many aspects of our lives.   COVID-19 has hobbled economies, strained societies and undermined the prospects of the next generation. While children are not at greatest direct risk from the virus itself, they continue to suffer disproportionately from its socioeconomic consequences. Almost two years into the pandemic, a generation of children are enduring prolonged school closures and ongoing disruptions to health, protection and education services.  

That is why today I am here to discuss the health threats facing the 2.2 billion children around the world who UNICEF serves, and the opportunity we have to protect them.  

Driven by new variants of concern, the virus continues to spread. While successful vaccination campaigns in the wealthy world have driven down rates of hospitalization and death, millions in low income countries await their first dose, and fragile health systems – on which children rely – are in jeopardy.  

Yet the gap between those who have been offered vaccination against COVID-19 and those who have not is widening. While some countries have protected most of their populations, in others, less than 3 per cent of the population have had their first dose. Those going without vaccines include doctors, midwives, nurses, community health workers, teachers and social workers – the very people that children, mothers and families rely upon for the most essential services.  

This is unacceptable. As a community of global health leaders, we have a choice. We can choose to act to reach more people with vaccines. This will keep people safe AND help to sustain critical services and systems for children.  

Today, almost 7 billion doses of COVID-19 vaccine have been administered, less than a year since the first vaccine was approved. And we are now on track to produce enough vaccines to protect the majority of people around the world before the end of next year.  

But will we protect everyone?   

Will we send lifesaving, health-system-saving COVID-19 vaccines to the world’s doctors, nurses, and most at-risk populations?  

Will donors continue to fund ACT-A and COVAX sufficiently to procure and successfully deploy the tests, treatments and vaccines needed to end the pandemic? Or will the costs of in-country delivery fall on struggling economies so that they are forced to cut other lifesaving health programmes such as routine childhood vaccinations? 

Will we stand by as the lowest-income countries, with the most fragile health systems, carry on unprotected – risking high death rates due to shortages of tests, treatments and vaccines? Or will we invest so that community health systems everywhere can withstand further waves of the virus, and bounce back from future shocks?  

Will we allow new variants of the virus to flourish in countries with low vaccination rates? Or, will we reap the benefits of global cooperation to defeat this global problem, together?   

The world has learned that financing for prevention, preparedness and response is insufficient and not adequately coordinated. And that is a vital lesson.  

But even more fundamentally, we have learned that the underlying strength of the health sector in general is a critical factor in a country’s ability to weather a storm like COVID-19.  

After all, what good are vaccines if there is no functioning public health system to deliver them?  

How do we hope to contain outbreaks if there are not enough trained and paid healthcare workers?  

This pandemic has been crippling for high income countries where average spending on healthcare per capita exceeds $5,000. So, it is hardly surprising that it is causing critical strain in lower-income countries where the average per capita expenditure on healthcare each year is less than $100.  

The past 22 months have shown us that even as we battle immediate threats such as a pandemic, we must also ensure continuous access to essential health services. If we do not, there will be an indirect increase in morbidity and mortality.  

As COVID-19 took hold of the world, healthcare workers serving pregnant mothers, babies and children faced unthinkable choices. As COVID patients gasped for breath, desperate for oxygen, mothers and babies needed it too. As wards filled up with virus victims, staff were not free to help the very young. As health budgets were stretched to the breaking point, routine healthcare began to go by the wayside.   

These are some of the reasons why more than twice as many women and children have lost their lives for every COVID-19 death in many low and middle-income countries. Estimates from the Lancet suggest up to nearly 114,000 additional women and children died during this period.  

I greatly fear that the pandemic’s impact on children’s health is only starting to be seen.  

While the pandemic has underscored that vaccination is one of the most cost-effective public health interventions, we have already seen backsliding in routine immunization. In 2020, over 23 million children missed out on essential vaccines – an increase of nearly 4 million from 2019, with decades of progress tragically eroded.  

Of these 23 million, 17 million of them did not receive any vaccines at all. These are the so-called zero-dose children, most of whom live in communities with multiple deprivations.       

Here are some of the most urgent choices we could make to address these problems: 

Governments can share COVID-19 doses with COVAX as a matter of absolute urgency and resist the temptation to stockpile supplies more than necessary.  

Governments can also honour their commitments to equitable access and make space for COVAX and other parts of ACT-A at the front of the supply queue for tests, treatments, and vaccines as they roll off production lines.  

Manufacturers can be more transparent about their production schedules and make greater efforts to facilitate and accelerate equitable access to products. This will help to ensure that COVAX and ACT-A get supplies faster. 

Governments, development banks, business and philanthropy can target strategic, sustainable investments in building robust and resilient primary healthcare services – embedded in each and every community.  

We can and we must choose a path ahead that is equitable, sustainable and rooted in the principle that every human being, young and old, rich and poor, has the right to good health.  

And there is good reason to believe that now is the time to set ourselves upon that path.  

A look back at history shows us that global threats and crises that challenge multiple interests and equities have a way of pulling together diverse partners to solve shared problems. Indeed, it is out of some of the most tragic crises that the world has found some of the best solutions.  

I believe now is such a time. We have a historic opportunity to both end the COVID-19 pandemic and set out on the road towards eradicating preventable diseases, ending avoidable maternal, newborn and child deaths, and building a strong foundation for community health that will serve this generation and the next.  

We can and we must seize this moment together.

Also, we are more addicted to eating junk foods and ordering food from outside. Therefore, this has given rise to several diseases starting from cardiovascular diseases, obesity, depression and anxiety.

Although there have been several medical technologies and treatments that can get you out of it, all it takes to stop this thing is self-control and a healthy lifestyle. As the saying goes, ‘Health is Wealth’, an individual can only enjoy all the valuables of life if he/she/they is both physically and mentally healthy. The health of an individual plays a major role in his/her/their life and must be taken care of. In spite of our busy schedules, it is our responsibility to prioritize our health and involve ourselves in some kind of physical activity. Physical exercises need not be always strenuous, it can also be done in the form of walking, cycling, yoga and meditation that brings calmness to both mind and body. 

But, is doing physical activity enough to stay healthy? Well, no. Most of the role to keep us healthy is played by the food we intake. It will be of no use if we intake junk and unhealthy food, rich in sugar and cholesterol levels and try to be healthy. Apart from exercising, it is equally important to drink plenty of water to keep ourselves hydrated. The level of fitness totally depends on the amount of activity our body can perform and the food we consume. Thus, inculcating a habit of eating healthy foods such as fruits, green vegetables and essential vitamins is really important.

How the brain produces speech

At a glance.

  • Researchers identified how neurons in the human brain encode various elements of speech.
  • The findings might be used to help develop treatments for speech and language disorders.

Speech and language depend on our ability to produce a wide variety of sounds in a specific order. How the neurons in the human brain work together to plan and produce speech remains poorly understood.

To begin to address this question, an NIH-funded team of researchers, led by Drs. Ziv Williams and Sydney Cash at Massachusetts General Hospital, recorded neuron activity during natural speech in five native English speakers. The experiments were done while participants were having electrodes implanted for deep brain stimulation. The researchers recorded neurons in a prefrontal brain region known to be involved in word planning and sentence construction. They used high-density arrays of tiny electrodes that could record signals from many individual neurons at once. Their results appeared in Nature on January 31, 2024.

The scientists found that the activity of almost half the neurons depended on the particular sounds, or phonemes, in the word about to be said. Some neurons, for instance, became more active ahead of speaking the sounds for “p” or “b”, which involve stopping airflow at the lips. Others did so ahead of speaking “k” or “g” sounds, which are formed by the tongue against the soft palate. Moreover, certain neurons seemed to reflect the specific combination of phonemes in the upcoming word. The team found that they could predict the phonemes that made up the word about to be spoken based on the activity of these neurons.

For about a quarter of the neurons, activity further reflected specific syllables, or ordered sequences of phonemes that may be all or part of a word. The team could predict the syllables in the upcoming word using the activity from these neurons. These neurons did not respond to the phonemes in the syllable by themselves. Nor did they respond to the phonemes out of order or split across different syllables.

A minority of neurons responded to the presence of prefixes or suffixes. These are examples of morphemes, or groups of sounds that carry specific meanings. The presence of morphemes in the upcoming word could be predicted from these neurons’ activities.

The team also found that different sets of neurons activated in a specific order. The morpheme neurons activated first, around 400 milliseconds (ms) before the utterance. Phoneme neurons activated next, around 200 ms before the utterance. Syllable neurons activated last, around 70 ms before utterance. Most neurons responded to the same feature (phoneme, syllable, or morpheme) both before and during the utterance. But the activity patterns during the utterance differed from those before it. Finally, the team found that neurons that responded to speech sounds during speaking differed from those that responded to those same speech sounds during listening.

In an accompanying paper in the same issue of Nature , another research team used the same technique to examine how neurons in another area of the brain respond while listening to speech. They similarly found that single neurons encoded different speech sound cues.

The findings suggest how various elements of speech are encoded in the brain, and how the brain combines these elements to form spoken words. This information might aid in developing brain-machine interfaces that can synthesize speech. Such devices could help a range of patients with conditions that impair speech.

“Disruptions in the speech and language networks are observed in a wide variety of neurological disorders—including stroke, traumatic brain injury, tumors, neurodegenerative disorders, neurodevelopmental disorders, and more,” says co-author Dr. Arjun Khanna. “Our hope is that a better understanding of the basic neural circuitry that enables speech and language will pave the way for the development of treatments for these disorders.”

Politics of language: jason stanley on speech as ‘hustle’.

Since the publication of his 2018 bestseller, “How Fascism Works: The Politics of Us and Them,” Yale’s Jason Stanley has become a familiar presence on radio and television news broadcasts. The Jacob Urowsky Professor of Philosophy in Yale’s Faculty of Arts and Sciences, Stanley is often called upon to explain the ways in which political language is being weaponized, especially when it seems to be for authoritarian ends.

“ There are very few philosophers in the media — people are usually historians or economists or psychologists,” Stanley said. “But I think philosophy has something to add.”

Stanley’s new book, “ The Politics of Language ” (Princeton University Press), which he co-authored with David Beaver, a professor of linguistics and philosophy at the University of Texas at Austin, argues that all speech is imbued with meanings that go well beyond what is being plainly said. Words evoke certain emotions and images in listeners depending on their historical associations. And those associations can be manipulated by politicians and spinmeisters for persuasive and possibly deceptive purposes.

A scholarly work eight years in the writing, “The Politics of Language” lays the intellectual foundation for this particular framing of speech practices, and then applies that framework to analyze harmful speech types such as slurs, “dog whistles,” and genocidal messaging.

Stanley spoke to Yale News about sneaky speech, the political power of plausible deniability, and his new teaching appointment in Ukraine.  

The primary focus of your book is a type of speech you call “hustle.” What is hustle?

Jason Stanley: Hustle is when you intend and give one message, but wrapped inside that message, as it were, is a sort of secret, other message that is not straightforward to your audience — and sometimes, not even straightforward to you. Straight talk is when everything’s transparent. You know what I intend, I know what you intend, and that’s all that’s going on. Hustle is everything else.

In linguistics, we assume transparency. The model of communication we assume is, “I want salt,” and you say, “I have salt,” and I say, “Good, can I have your salt?” But actually, communication isn’t like that at all, right? We always come out of conversations wondering what was really meant. A central novel move in our book, especially for the formal areas of linguistics and philosophy of language in which we work, is that we base our analysis of speech on speech practices  and use these practices to elucidate speech that is not transparent. A lot of hustle is based on the properties of the speech practices in which words are embedded — their histories of use. I think that everyone outside of the disciplines of linguistics and analytic philosophy of language is aware that we hustle, and many people outside of these disciplines recognize the importance of speech practices. But we are trying to ground these facts in a detailed and foundational way.

My 2015 book, “ How Propaganda Works ,” was an attempt to use the standard tools of philosophy of language and linguistics to model propaganda. These tools just didn’t work. They also don’t work for more ordinary phenomena, such as slurs. That led me to enlist the support of the great linguist David Beaver to come up with new foundations for the theory of meaning that would be adequate to this task. To do so, we had to bridge multiple disciplines.

Stanley: Speech can be used in devious ways, and there are tests you can use to show this. One test is plausible deniability. If I say, “I met Sally at the mathematics conference,” I cannot add, “and I never met Sally.” That’s an obvious contradiction. Now, suppose that a politician says, “There is rampant corruption in inner cities.” The politician is engaging in racist messaging, using a dog whistle — in this case, the term “inner cities.” They are suggesting that the voting practices of Black voters are corrupted. But they can deny, without obvious contradiction, that they intended to convey a racist message. The tools of our disciplines have a hard time explaining this.

Another example is emotion. Some expressions are associated with negative emotions, like disgust. Politicians even try to imbue certain words with disgust. The scholar Moira Weigel has argued that this is going on now with the term “Marxist .” Other words encourage violence towards things described that way, such as describing immigrants as “vermin.” Ordinary tools in our discipline are not adequate to explain these phenomena, which is why you haven’t seen many analytic philosophers of language or linguists on TV in the past few years. If you recognize the embeddedness of speech in living practices, you can easily explain all of this.

This non-transparency of speech practices makes all speech “political” then?  

Stanley: Each word is part of a speech practice and has a history. And when you use a given word, you’re evoking that history in peoples’ minds. If you use the word “professor,” a whole bunch of images come to mind. And they’re involuntary. If you use the word “doggy,” one set of images comes to mind. If you use the word “canine,” those same images don’t come to mind. Words have these histories and the words we use consciously and unconsciously evoke those histories. So when we speak one way, rather than another, we’re evoking different histories. It’s a fiction to think we can speak without a history. In much theorizing in our disciplines we have employed this fiction as an idealization, and it has shielded from us a lot of the interest of speech.

Politicians use language very deliberately to evoke certain images. How does repetition of language function in political speak and propaganda?

Stanley: It sort of embeds the propaganda in your head. If Republicans keep repeating CRT [for critical race theory] or DEI [for diversity, equity, and inclusion], and they associate them with a kind of negative feeling, then after a while, the listeners don’t even think about what they mean. They just know they don’t like them. The content becomes irrelevant. Consider how Trump adds those adjectives before nouns, like “crooked Hillary” or “little Marco Rubio.” It’s a very effective thing, right? Or take pro-life versus pro-choice. Who’s against life? Who’s against choice? That’s a different way of coding.

One of your chapters focuses on “harmful speech,” including a discussion of genocidal speech. You offer as a prime example Russia’s justification for its invasion of Ukraine. Would you talk about that?

Stanley: It’s impossible to deny that Russia is appealing to genocidal narratives. When you represent other people as an existential threat to you, that you need to eradicate them or you will be eradicated, that’s genocidal and has been genocidal since the time of Cleon. Putin says Ukrainian identity is fake, and that all it means is “anti-Russia.” There’s no other content to Ukrainian identity other than, “we hate Russians.” And so you must extinguish Ukrainian identity.

What’s distinct about what’s happening in the Ukraine war is that the Russians think the Ukrainians really are Russian but have adopted this anti-Russian identity and a fake language.

You have a close connection with Ukraine, don’t you?

Stanley: I was there in August teaching a two-week course on colonialism and fascism to 300 Ukrainians. And I just accepted a permanent visiting position in Kyiv, so I’ll be going back this summer for two weeks. I’m an anti-fascist, and that is a very clear case of a fascist country attacking a democracy, so I felt I had to be involved. I’m donating my salary to Come Back Alive, an organization that purchases equipment for the Ukrainian armed forces.

Is the timing of this book especially meaningful given all that’s happening politically in the world?

Stanley: Liberal democracy involves this idea that we’re trading arguments to figure out the best outcome for all of us. But that’s not what’s going on in politics now. To understand this authoritarian moment here and across the world, this attack on democracy, it’s also a linguistic attack. It’s emphasizing the uses of speech that are sneaky, that are hustle.

Last week, when a civil court judge in New York ruled against Donald J. Trump, he imposed a set of penalties so severe that they could temporarily sever the former president from his real-estate empire and wipe out all of his cash.

Jonah Bromwich, who covers criminal justice in New York, and Maggie Haberman, a senior political correspondent for The Times, explain what that will mean for Mr. Trump as a businessman and as a candidate.

2023 BC Adolescent Health Survey Results Released

On Wednesday, February 21, 2024 The McCreary Centre Society released the newest iteration of the BC Adolescent Health Survey. The survey was taken in 2023 by 38,500 public school students from Grade 7 to Grade 12. Conducted every five years since 1992, this invaluable research tool provides data about mental health, alcohol and drug consumption, sexual activity and other topics of concern for youth.

An article in The Globe and Mail gives examples of some of the most recent trends: "The survey results show there has been a decrease over the past 30 years in the percentage of youth who had ever smoked tobacco. In 2023, 15 per cent had smoked tobacco, compared to 21 per cent a decade ago, 34 per cent two decades ago and 60 per cent in 1992... A similar pattern was found in alcohol consumption." 

But more recently, "compared to five years ago, there was a slight increase in youth who tried cannabis for the first time at age 12 or younger, and a decrease in the percentage who waited until they were 15 or older to try it."

According to Dr. Elizabeth Saewyc , research director for the BC Adolescent Health Survey project team, the legalization of cannabis has not been the impetus for the trend toward an earlier age of first-time users. Pointing to the similar results in the report in relation to alcohol and tobacco, she notes, “These two have been legal for much longer than cannabis (although still not legal for adolescents). If this change were due to cannabis legalization, it should only show up as a trend for cannabis, not for both alcohol or tobacco.”

The survey results also indicated that youth were less likely to have engaged in sexual activity, reported poorer mental health, indicated more cases of self-harm and were less hopeful for their future than teens were five years ago. They also sleep less, feel less safe at school, are less likely to feel connected to their school and community, and are less likely to have in-person friends.

The survey results contain a wealth of data that researchers will tap into to provide further insights on teen health trends for years to come.

Freedom of Speech Not Reach: New Updates and Progress

On X people are free to be their true selves. Everyday, we work to preserve free speech on X, while equally maintaining the health of our platform. Since the launch of Freedom of Speech Not Reach , we’ve seen encouraging results. That's why in the coming weeks, we’re expanding the application of this enforcement action from our Hateful Conduct policy to now include our policies on Abusive Behavior and Violent Speech . 

Today, more than 99.99% of post impressions are from healthy content, or content that does not violate our rules. For the small minority of content that is violative, we apply proportionally appropriate enforcement action. We will continue to remove the most serious violations of our rules, such as illegal content, and suspend bad actors from our platform. 

More than any other platform, X's enforcement approach brings a new level of transparency to restricting the reach of posts identified as potentially violating our policies by making the content less discoverable.

The X community has also provided valuable feedback to help us make meaningful changes to the accuracy of our label application, such as identifying instances where reach was not appropriately restricted and improving recognition of context in our detection.

Here is some of the impact since we launched this new enforcement philosophy:

  • Labels have been applied to more than 700,000 violative posts that fall under our Hateful Conduct policy. We also proactively prevent ads from appearing adjacent to content that we label.
  • Compared to a healthy post, posts with these labels—or restricted posts— receive 81% less reach or impressions and we proactively prevent ads from appearing adjacent to content that we label.
  • More than one third of authors proactively choose to delete the post after they are informed that its reach has been restricted.
  • On average, only 4 percent of authors have appealed labels.
  • 90 percent of all appeals receive a response within 30 minutes.

While this progress is promising, our work is ongoing. And as part of our commitment to transparency and results, we’ll continue to provide periodic updates on our progress.
