Mental Health Month has been observed to reduce the stigma that is associated with mental illness and to educate the public and encourage individuals to make their mental health and wellbeing a priority. It is an important moment to bring the strengths of advocacy groups and researchers together to promote mental health awareness and to improve equity.

Observances have become a popular tool to garner media and notice for topics deserving attention, from medical conditions to public health concerns, commemoration of notable events, or celebration of cultural groups. Codifying the scope and needs connected to an issue or illness through awareness campaigns can provide opportunities for imparting useful information, reducing stigma and marshalling support for policy change. The impact of awareness campaigns can be difficult to measure beyond tallying social media mentions or news stories. Effective advocacy, however, extends beyond traffic and paves the way for the creation of knowledge and partnerships among allies and with those whose interests are being represented. When the magnitude of an issue and the potential for improvement are great and are matched by broad involvement and recognition by stakeholders, the possibility for impact is also great.

Held annually in May, Mental Health Month , also called Mental Health Awareness Month, is an observance with such reach, resonating with many people. Nearly everyone has experience with the challenges that are associated with mental health, either first-hand or through loved ones or in their community. There is a need for education, support and initiative to improve our understanding of the causes of mental health disorders and to increase the availability of resources for prevention and treatment. Mental Health Month also offers the possibility of bringing together groups who often work in parallel, such as mental health advocates and mental health research organizations, that can mutually benefit from each other’s functions and expertise.

Mental Health Month was first established in the US in 1949 by the National Mental Health Association, now known as Mental Health America . At a time more often associated with the outset of the Cold War and Marshall Plan than setting an agenda for domestic mental health and wellbeing advocacy, in the more than 70 years since, Mental Health Month has grown into an international event designed to reduce exclusion, stigma and discrimination against people with mental health conditions or disorders. Mental Health America are joined by other prominent mental health advocacy groups to sponsor related observances: Mental Health Awareness Week Canada (1–7 May, 2023) and Europe (22–28 May, 2023); and federal agencies such as the Substance Use Abuse and Mental Health Services Administration ( SAMSHA ) in the US, promoting related public education platforms, including National Prevention Week (7–13 May, 2023).

Observances and awareness campaigns also provide occasions to put mental health in context. Increasing acknowledgment of the role of social determinants, for example, as mechanisms that can increase vulnerability for developing disorders and that drive disparities in mental health are an important framework to underscore as part of promoting mental health awareness. Given the complex and broad scope of people, disorders, conditions and issues under the umbrella of mental health, observances also give us the chance to focus more closely on specific problems or experiences. The theme for Mental Health Month in 2023 is ‘Look Around, Look Within’, which emphasizes the interdependence of mental health and wellness with an individual’s internal and external experiences and environments.

“The ‘Look Around, Look Within’ theme builds on the growing recognition that all humans have mental health needs and that our available resources to build resilience and heal come in many forms — including in the natural world,” explains Jennifer Bright, Mental Health America Board Chair and President of Momentum Health Strategies. “Mental Health America’s strategic plan, focused on NextGen Prevention, carries a similar theme — that the social factors supporting mental health are essential building blocks. These encompass basic needs like healthy food, stable housing, and access to treatment and supports, but they also include spirituality, connection with peers with lived experience, and safe and natural spaces.”

Overlapping with Mental Health Month, Mental Health Foundation sponsors Mental Health Week in the UK (15–21 May, 2023), dedicating this year to raising awareness around anxiety. It shares an individual-centered approach to advocacy. In addition to providing toolkits and resources that point to how prevalent stress and anxiety can be to reduce stigma, it also promotes the accessibility of coping strategies for managing anxiety. As part of the Mental Health Awareness Week campaign, Mental Health Foundation and others use the international symbol of wearing a green ribbon or clothing to physically raise awareness around mental health. Nature Mental Health also incorporates the symbol of the green ribbon on the cover of this month’s issue and as our journal theme color. Green evokes the ideas of vitality, growth, new beginnings and hope — powerful imagery in mental health awareness.

Alongside stories, sponsorships and social media resources, mental health advocacy toolkits and strategy documents include fact sheets and messaging that are shaped and informed by research. Yet, there is often a perception that a divide exists between the mental health advocacy and research spaces, but observances such as Mental Health Month can bridge the two.

According to Lea Milligan, Chief Executive Officer of MQ: Transforming Mental Health , an international mental health research organization, there are complementary approaches and priorities in advocacy and research: “Mental health research can be used to bolster awareness by providing evidence-based information and resources that can help individuals and communities better understand mental health and the factors that contribute to mental health problems. This can include information on risk factors, prevention strategies, and available treatments.”

In addition, increased efforts to involve people with lived experience of mental illness in the research enterprise is a goal that is well-served through connection with advocacy. “While MQ is primarily focused on promoting mental health research, it also recognizes the importance of advocacy in advancing the mental health agenda” suggests Milligan. “MQ advocates for increased funding and support for mental health research, as well as policies that promote mental health and wellbeing. Additionally, MQ seeks to empower individuals with lived experience of mental health conditions to be involved in research and advocacy efforts, and to have their voices heard in the development of policies and programs that affect their lives. MQ provides resources and support for individuals with lived experience who wish to be involved in research or advocacy efforts, including training programs, research grants, and opportunities to participate in research studies.”

Involvement or engagement is certainly one of the most important metrics of advocacy. By strengthening collaboration between advocacy and research organizations and identifying the mutual areas of benefit, such as engagement and increased funding, we may find new ways to green light mental health awareness and action toward mental health equity.

Recent research indicates that understanding how children and youth perceive mental health, how it is manifests, and where the line between mental health issues and everyday challenges should be drawn, is complex and varied. Consequently, it is important to investigate how children and youth perceive and communicate about mental health. With this in mind, our goal is to synthesize the literature on how children and youth (ages 10—25) perceive and conceptualize mental health.

We conducted a preliminary search to identify the keywords, employing a search strategy across electronic databases including Medline, Scopus, CINAHL, PsychInfo, Sociological abstracts and Google Scholar. The search encompassed the period from September 20, 2021, to September 30, 2021. This effort yielded 11 eligible studies. Our scoping review was conducted in accordance with the PRISMA-ScR Checklist.

As various aspects of uncertainty in understanding of mental health have emerged, the results indicate the importance of establishing a shared language concerning mental health. This is essential for clarifying the distinctions between everyday challenges and issues that require treatment.

We require a language that can direct children, parents, school personnel and professionals toward appropriate support and aid in formulating health interventions. Additionally, it holds significance to promote an understanding of the positive aspects of mental health. This emphasis should extend to the competence development of school personnel, enabling them to integrate insights about mental well-being into routine interactions with young individuals. This approach could empower children and youth to acquire the understanding that mental health is not a static condition but rather something that can be enhanced or, at the very least, maintained.

In Western society, the prevalence of mental health issues, such as depression and anxiety [ 1 ], as well as recurring psychosomatic health complaints [ 2 ], has increased from the 1980s and 2000s. However, whether these changes in adolescent mental health are actual trends or influenced by alterations in how adolescents perceive, talk about, and report their mental well-being remains ambiguous [ 1 ]. Despite an increase in self-reported mental health problems, levels of mental well-being have remained stable, and severe psychiatric diagnoses have not significantly risen [ 3 , 4 ]. Recent research indicates that understanding how children and youth grasp mental health, its manifestations, and the demarcation between mental health issues and everyday challenges is intricate and diverse. Wickström and Kvist Lindholm [ 5 ] show that problems such as feeling low and nervous are considered deep-seated issues among some adolescents, while others refer to them as everyday challenges. Meanwhile, adolescents in Hellström and Beckman [ 6 ] describe mental health problems as something mainstream, experienced by everyone at some point. Furthermore, Hermann et al. [ 7 ] point out that adolescents can distinguish between positive health and mental health problems. This indicates their understanding of the complexity and holistic nature of mental health and mental health issues. It is plausible that misunderstandings and devaluations of mental health and illness concepts may increase self-reported mental health problems and provide contradictory results when the understanding of mental health is studied. In a previous review on how children and young people perceive the concept of “health,” four major themes have been suggested: health practices, not being sick, feeling good, and being able to do the desired and required activities [ 8 ]. In a study involving 8–11 year olds, children framed both biomedical and holistic perspectives of health [ 9 ]. Regarding the concept of “illness,” themes such as somatic feeling states, functional and affective states [ 10 , 11 ], as well as processes of contagion and contamination, have emerged [ 9 ]. Older age strongly predicts nuances in conceptualizations of health and illness [ 10 , 11 , 12 ].

As the current definitions of mental health and mental illness do not seem to have been successful in guiding how these concepts are perceived, literature has emphasized the importance of understanding individuals’ ideas of health and illness [ 9 , 13 ]. The World Health Organization (WHO) broadly defines mental health as a state of well-being in which the individual realizes his or her abilities, can cope with the normal stresses of life, work productively and fruitfully and make a contribution to his or her community [ 14 ] capturing only positive aspects. According to The American Psychology Association [ 15 ], mental illness includes several conditions with varying severity and duration, from milder and transient disorders to long-term conditions affecting daily function. The term can thus cover everything from mild anxiety or depression to severe psychiatric conditions that should be treated by healthcare professionals. As a guide for individual experience, such a definition becomes insufficient in distinguishing mental illness from ordinary emotional expressions. According to the Swedish National Board of Health and Welfare et al. [ 16 ], mental health works as an umbrella term for both mental well-being and mental illness : Mental well-being is about being able to handle life's difficulties, feeling satisfied with life, having good social relationships, as well as being able to feel pleasure, desire, and happiness. Mental illness includes both mild to moderate mental health problems and psychiatric conditions . Mild to moderate mental health problems are common and are often reactions to events or situations in life, e.g., worry, feeling low, and sleep difficulties.

It has been argued that increased knowledge of the nature of mental illness can help individuals to cope with the situation and improve their well-being. Increased knowledge about mental illness, how to prevent mental illness and help-seeking behavior has been conceptualized as “mental health literacy” (MHL) [ 17 ], a construct that has emerged from “health literacy” [ 18 ]. Previous literature supports the idea that positive MHL is associated with mental well-being among adolescents [ 19 ]. Conversely, studies point out that low levels of MHL are associated with depression [ 20 ]. Some gender differences have been acknowledged in adolescents, with boys scoring lower than girls on MHL measures [ 20 ] and a social gradient including a positive relationship between MHL and perceived good financial position [ 19 ] or a higher socio-economic status [ 21 ].

While MHL stresses knowledge about signs and treatment of mental illness [ 22 ], the concern from a social constructivist approach would be the conceptualization of mental illness and how it is shaped by society and the thoughts, feelings, and actions of its members [ 23 ]. Studies on the social construction of anxiety and depression through media discourses have shown that language is at the heart of these processes, and that language both constructs the world as people perceive it but also forms the conditions under which an experience is likely to be construed [ 24 , 25 ]. Considering experience as linguistically inflected, the constructionist approach offers an analytical tool to understand the conceptualization of mental illness and to distinguish mental illness from everyday challenges. The essence of mental health is therefore suggested to be psychological constructions identified through how adolescents and society at large perceive, talk about, and report mental health and how that, in turn, feeds a continuous process of conceptual re-construction or adaptation [ 26 ]. Considering experience as linguistically inflected, the constructionist approach could then offer an analytical tool to understand the potential influence of everyday challenges in the conceptualization of mental health.

Research investigating how children and youth perceive and communicate mental health is essential to understand the current rise of reported mental health problems [ 5 ]. Health promotion initiatives are more likely to be successful if they take people’s understanding, beliefs, and concerns into account [ 27 , 28 ]. As far as we know, no review has mapped the literature to explore children’s and youths’ perceptions of mental health and mental illness. Based on previous literature, age, gender, and socioeconomic status seem to influence children's and youths’ knowledge and experiences of mental health [ 10 , 11 , 12 ]; therefore, we aim to analyze these perspectives too. From a social constructivist perspective, experience is linguistically inflected [ 26 ]; hence illuminating the conditions under which a perception of health is formed is of interest.

Therefore, we aim to study the literature on how children and youth (ages 10—25) perceive and conceptualize mental health, and the specific research questions are:

What aspects are most salient in children’s and youths’ perceptions of mental health?

What concepts do children and youth associate with mental health?

In what way are children's and youth’s perceptions of mental health dependent on gender, age, and socioeconomic factors?

Literature search

A scoping review is a review that aims to provide a snapshot of the research that is published within a specific subject area. The purpose is to offer an overview and, on a more comprehensive level, to distinguish central themes compared to a systematic review. We chose to conduct a scoping review since our aim was to clarify the key concepts of mental health in the literature and to identify specific characteristics and concepts surrounding mental health [ 29 , 30 ]. Our scoping review was conducted following the PRISMA-ScR Checklist [ 31 ]. Two authors (L.B and L.H) searched and screened the eligible articles. In the first step, titles and abstracts were screened. If the study included relevant data, the full article was read to determine if it met the eligibility criteria. Articles were excluded if they did not fulfill all the eligibility criteria. Any uncertainties were discussed among L.B. and L.H., and the third author, S.H., and were carefully assessed before making an inclusion or exclusion decision. The software Picoportal was employed for data management. Figure  1 illustrates a flowchart of data inclusion.

Eligibility criteria

We incorporated studies involving children and youth aged 10 to 25 years. This age range was chosen to encompass early puberty through young adulthood, a significant developmental period for young individuals in terms of comprehending mental health. Participants were required not to have undergone interviews due to chronic illness, learning disabilities (e.g., mental health linked to a cancer diagnosis), or immigrant status.

Studies conducted in clinical settings were excluded. For the purpose of comparing results under similar conditions, we specifically opted for studies carried out in Western countries .

Given that this review adopts a moderately constructionist approach, intentionally allowing for the exploration of how both young participants and society in general perceive and discuss mental health and how this process contributes to ongoing conceptual re-construction, the emphasis was placed on identifying articles in which participants themselves defined or attributed meaning to mental health and related concepts like mental illness. The criterion of selecting studies adopting an inductive approach to capture the perspectives of the young participants resulted in the exclusion of numerous studies that more overtly applied established concepts to young respondents [ 32 ].

Information sources

We utilized electronic databases and reached out to study authors if the article was not accessible online. Peer-reviewed articles were exclusively included, thereby excluding conference abstracts due to their perceived lack of relevance in addressing the review questions. Only research in English was taken into account. Publication years across all periods were encompassed in the search.

Search strategy

Studies concerning children’s and youths’ perceptions of mental health were published across a range of scientific journals, such as those within psychiatry, psychology, social work, education, and mental health. Therefore, several databases were taken into account, including Medline, Scopus, CINAHL, PsychInfo, Sociological abstracts, and Google Scholar, spanning from inception on September 20, 2021 to September 30, 2021. We involved a university librarian from the start in the search process. The combinations of search terms are displayed in Table 1 .

Quality assessment

We employed the Quality methods for the development of National Institute for Health Care Excellence (NICE) public health guidance [ 33 ] to evaluate the quality of the studies included. The checklist is based on checklists from Spencer et al. [ 34 ], Public Health Resource Unit (PHRU) [ 26 , 35 ], and the North Thames Research Appraisal Group (NTRAG) [ 36 ] (Refer to S2 for checklist). Eight studies were assigned two plusses, and three studies received one plus. The studies with lower grades generally lacked sufficient descriptions of the researcher’s role, context reporting, and ethical reporting. No study was excluded in this stage.

Data extraction and analysis

We employed a data extraction form that encompassed several key characteristics, including author(s), year, journal, country, details about method/design, participants and socioeconomics, aim, and main results (Table 2 ). The collected data were analyzed and synthesized using the thematic synthesis approach of Thomas and Harden [ 37 ]. This approach encompassed all text categorized as 'results' or 'findings' in study reports – which sometimes included abstracts, although the presentation wasn’t always consistent throughout the text. The size of the study reports ranged from a few sentences to a single page. The synthesis occurred through three interrelated stages that partially overlapped: coding of the findings from primary studies on a line-by-line basis, organization of these 'free codes' into interconnected areas to construct 'descriptive' themes, and the formation of 'analytical' themes.

The objective of this scoping review has been to investigate the literature concerning how children and youth (ages 10—25) conceptualize and perceive mental health. Based on the established inclusion- and exclusion criteria, a total of 11 articles were included representing the United Kingdom ( n  = 6), Australia ( n  = 3), and Sweden ( n  = 2) and were published between 2002 and 2020. Among these, two studies involved university students, while nine incorporated students from compulsory schools.

Salient aspects of children and youth’ perceptions of mental health

Based on the results of the included articles, salient aspects of children’s and youths’ understandings revealed uncertainties about mental health in various ways. This uncertainty emerged as conflicting perceptions, uncertainty about the concept of mental health, and uncertainty regarding where to distinguish between mild to moderate mental health problems and everyday stressors or challenges.

One uncertainty was associated with conflicting perceptions that mental health might be interpreted differently among children and youths, depending on whether it relates to their own mental health or someone else's mental health status. Chisholm et al. [ 42 ] presented this as distinctions being made between ‘them and us’ and between ‘being born with it’. Mental health and mental illness were perceived as a continuum that rather developed’, and distinctions were drawn between ‘crazy’ and ‘diagnosed.’ Participants established strong associations between the term mental illness and derogatory terms like ‘crazy,’ linking extreme symptoms of mental illness with others. However, their attitude was less stigmatizing when it came to individual diagnoses, reflecting a more insightful and empathetic understanding of the adverse impacts of stress based on their personal realities and experiences. Despite the initial reactions reflecting negative stereotypes, further discussion revealed that this did not accurately represent a deeper comprehension of mental health and mental illness.

There was also uncertainty about the concept of mental health , as it was not always clearly understood among the participating youth. Some participants were unable to define mental health, often confusing it with mental illness [ 28 ]. Others simply stated that they did not understand the term, as in O’Reilly [ 44 ]. Additionally, uncertainty was expressed regarding whether mental health was a positive or negative concept [ 27 , 28 , 40 , 44 ], and participants associated mental health with mental illness despite being asked about mental health [ 28 ]. One quote from a grade 9 student illustrates this: “ Interviewer: Can mental health be positive as well? Informant: No, it’s mental” [ 44 ]. In Laidlaw et al. [ 46 ], with participants ranging from 18—22 years of age, most considered mental health distinctly different from and more clinical than mental well-being. However, Roose et al. [ 38 ], for example, the authors discovered a more multifaceted understanding of mental health, encompassing emotions, thoughts, and behavior. In Molenaar et al.[ 45 ], mental health was highlighted as a crucial aspect of health overall. In Chisholm et al. [ 42 ], the older age groups discussed mental health in a more positive sense when they considered themselves or people they knew, relating mental health to emotional well-being. Connected to the uncertainty in defining the concept of mental health was the uncertainty in identifying those with good or poor mental health. Due to the lack of visible proof, children and youths might doubt their peers’ reports of mental illness, wondering if they were pretending or exaggerating their symptoms [ 27 ].

A final uncertainty that emerged was difficulties in drawing the line between psychiatric conditions and mild to moderate mental health problems and everyday stressors or challenges . Perre et al. [ 43 ] described how the participants in their study were uncertain about the meaning of mental illness and mental health issues. While some linked depression to psychosis, others related it to simply ‘feeling down.’ However, most participants indicated that, in contrast to transient feelings of sadness, depression is a recurring concern. Furthermore, the duration of feeling depressed and particularly a loss of interest in socializing was seen as appropriate criteria for distinguishing between ‘feeling down’ and ‘clinical depression.’ Since feelings of anxiety, nervousness, and apprehension are common experiences among children and youth, defining anxiety as an illness as opposed to an everyday stressor was more challenging [ 43 ].

Terms used to conceptualize mental health

When children and youth were asked about mental health, they sometimes used neutral terms such as thoughts and emotions or a general ‘vibe’ [ 27 ], and some described it as ‘peace of mind’ and being able to balance your emotions [ 38 ]. The notion of mental health was also found to be closely linked with rationality and the idea of normality, although, according to the young people, Armstrong et al. [ 28 ], there was no consensus about what ‘normal’ meant. Positive aspects of mental health were described by the participants as good self-esteem, confidence [ 40 ], happiness [ 39 , 43 ], optimism, resilience, extraversion and intelligence [ 27 ], energy [ 43 ], balance, harmony [ 39 , 43 ], good brain, emotional and physical functioning and development, and a clear idea of who they are [ 27 , 41 ]. It also included a feeling of being a good person, feeling liked and loved by your parents, social support, and having people to talk with [ 27 , 39 ], as well as being able to fit in with the world socially and positive peer relationships [ 41 ], according to the children and youths, mental health includes aspects related to individuals (individual factors) as well as to people in their surroundings (relationships). Regarding mental illness, participants defined it as stress and humiliation [ 40 ], psychological distress, traumatic experiences, mental disorders, pessimism, and learning disabilities [ 27 ]. Also, in contrast to the normality concept describing mental health, mental illness was described as somehow ‘not normal’ or ‘different’ in Chisholm et al. [ 42 ].

Depression and bipolar disorder were the most often mentioned mental illnesses [ 27 ]. The inability to balance emotions was seen as negative for mental health, for example, not being able to set aside unhappiness, lying to cover up sadness, and being unable to concentrate on schoolwork [ 38 ]. The understanding of mental illness also included feelings of fear and anxiety [ 42 ]. Other participants [ 46 ] indicated that mental health is distinctly different from, and more clinical than, mental well-being. In that sense, mental health was described using reinforcing terms such as ‘serious’ and ‘clinical,’ being more closely connected to mental illness, whereas mental well-being was described as the absence of illness, feeling happy, confident, being able to function and cope with life’s demands and feeling secure. Among younger participants, a more varied and vague understanding of mental health was shown, framing it as things happening in the brain or in terms of specific conditions like schizophrenia [ 44 ].

Gender, age, socioeconomic status

Only one study had a gender theoretical perspective [ 40 ], but the focus of this perspective concerned gender differences in what influences mental health more than the conceptualization of mental health. According to Johansson et al.[ 39 ], older girls expressed deeper negative emotions (e.g., described feelings of lack of meaning and hope in various ways) than older boys and younger children.

Several of the included studies noticed differences in age, where younger participants had difficulty understanding the concept of mental health [ 39 , 44 ], while older participants used more words to explain it [ 39 ]. Furthermore, older participants seemed to view mental health and mental illness as a continuum, with mental illness at one end of the continuum and mental well-being at the other end [ 42 , 46 ].

Socioeconomic status

The role of socioeconomic status was only discussed by Armstrong et al. [ 28 ], finding that young people from schools in the most deprived and rural areas experienced more difficulties defining the term mental health compared to those from a less deprived area.

This scoping review aimed to map children's and youth’s perceptions and conceptualizations of mental health. Our main findings indicate that the concept of mental health is surrounded by uncertainty. This raises the question of where this uncertainty stems from and what it symbolizes. From our perspective, this uncertainty can be understood from two angles. Firstly, the young participants in the different studies show no clear and common understanding of mental health; they express uncertainty about the meaning of the concept and where to draw the line between life experiences and psychiatric conditions. Secondly, uncertainty exists regarding how to apply these concepts in research, making it challenging to interpret and compare research results. The shift from a positivistic understanding of mental health as an objective condition to a more subjective inner experience has left the conceptualization open ranging from a pathological phenomenon to a normal and common human experience [ 47 ]. A dilemma that results in a lack of reliability that mirrors the elusive nature of the concept of mental health from both a respondent and a scientific perspective.

“Happy” was commonly used to describe mental health, whereas "unhappy" was used to describe mental illness. The meaning of happiness for mental health has been acknowledged in the literature, and according to Layard et al. [ 48 ], mental illness is one of the main causes of unhappiness, and happiness is the ultimate goal in human life. Layard et al. [ 48 ] suggest that schools and workplaces need to raise more awareness of mental health and strive to improve happiness to promote mental health and prevent mental illness. On the other hand, being able to experience and express different emotions could also be considered a part of mental health. The notion of normality also surfaced in some studies [ 38 ], understanding mental health as being emotionally balanced or normal or that mental illness was not normal [ 42 ]. To consider mental illness in terms of social norms and behavior followed with the sociological alternative to the medical model that was introduced in the sixties portraying mental illness more as socially unacceptable behavior that is successfully labeled by others as being deviant. Although our results did not indicate any perceptions of what ‘normal’ meant [ 28 ], one crucial starting point to the understanding of mental health among adolescents should be to delineate what constitutes normal functioning [ 23 ]. Children and youths’ understanding of mental illness seems to a large extent, to be on the same continuum as a normality rather than representing a medicalization of deviant behavior and a disjuncture with normality [ 49 ].

Concerning gender, it seemed that girls had an easier time conceptualizing mental health than boys. This could be due to the fact that girls mature verbally faster than boys [ 50 ], but also that girls, to a larger extent, share feelings and problems together compared to boys [ 51 ]. However, according to Johansson et al. [ 39 ], the differences in conceptualizations of mental health seem to be more age-related than gender-related. This could be due to the fact that older children have a more complex view of mental health compared to younger children.. Not surprisingly, the older the children and youth were, the more complex the ability to conceptualize mental health becomes. Only one study reported socioeconomic differences in conceptualizations of mental health [ 28 ]. This could be linked to mental health literacy (MHL) [ 18 ], i.e., knowledge about mental illness, how to prevent mental illness, and help-seeking behavior. Research has shown that disadvantaged social and socioeconomic conditions are associated with low MHL, that is, people with low SES tends to know less about symptoms and prevalence of different mental health problems [ 19 , 21 ]. The perception and conceptualizations of mental health are, as we consider, strongly related to knowledge and beliefs about mental health, and according to von dem Knesebeck et al. [ 52 ] linked primarily to SES through level of education.

Chisholm et al. [ 42 ] found that the initial reactions from participants related to negative stereotypes, but further discussion revealed that the participants had more refined knowledge than at first glance. This illuminates the importance of talking to children and helping them verbalize their feelings, in many respects complex and diversified understanding of mental health. It is plausible that misunderstandings and devaluations of mental health and mental illness may increase self-reported mental health problems [ 5 ], as well as decrease them, preventing children and youth from seeking help. Therefore, increased knowledge of the nature of mental health can help individual cope with the situations and improve their mental well-being. Finding ways to incorporate discussions about mental well-being, mental health, and mental illness in schools could be the first step to decreasing the existing uncertainties about mental health. Experiencing feelings of sadness, anger, or upset from time to time is a natural part of life, and these emotions are not harmful and do not necessarily indicate mental illness [ 5 , 6 ]. Adolescents may have an understanding of the complexity of mental health despite using simplified language but may need guidance on how to communicate their feelings and how to manage everyday challenges and normal strains in life [ 7 ].

With the aim of gaining a better understanding of how mental health is perceived among children and youth, this study has highlighted the concept’s uncertainty. Children and youth reveal a variety of understandings, from diagnoses of serious mental illnesses such as schizophrenia to moods and different types of behaviors. Is there only one way of understanding mental health, and is it reasonable to believe that we can reach a consensus? Judging by the questions asked, researchers also seem to have different ideas on what to incorporate into the concept of mental health — the researchers behind the present study included. The difficulties in differentiating challenges being part of everyday life with mental health issues need to be paid closer attention to and seems to be symptomatic with the lack of clarity of the concepts.

A constructivist approach would argue that the language of mental health has changed over time and thus influence how adolescents, as well as society at large, perceive, talk about, and report their mental health [ 26 ]. The re-construction or adaptation of concepts could explain why children and youth re struggling with the meaning of mental health and that mental health often is used interchangeably with mental illness. Mental health, rather than being an umbrella term, then represents a continuum with a positive and a negative end, at least among older adolescents. But as mental health according to this review also incorporates subjective expressions of moods and feelings, the reconstruction seems to have shaped it into a multidimensional concept, representing a horizontal continuum of positive and negative mental health and a vertical continuum of positive and negative well-being, similar to the health cross by Tudor [ 53 ] referred to in Laidlaw et al. [ 46 ] A multidimensional understanding of mental health constructs also incorporates evidence from interventions aimed at reducing mental health stigma among adolescents, where attitudes and beliefs as well as emotional responses towards mental health are targeted [ 54 ].

The contextual understanding of mental health, whether it is perceived in positive terms or negative, started with doctors and psychiatrists viewing it as representing a deviation from the normal. A perspective that has long been challenged by health workers, academics and professionals wanting to communicate mental health as a positive concept, as a resource to be promoted and supported. In order to find a common ground for communicating all aspects and dimensions of mental health and its conceptual constituents, it is suggested that we first must understand the subjective meaning ascribed to the use of the term [ 26 ]. This line of thought follows a social-constructionist approach viewing mental health as a concept that has transitioned from representing objective mental descriptions of conditions to personal subjective experiences. Shifting from being conceptualized as a pathological phenomenon to a normal and common human experience [ 47 ]. That a common understanding of mental health can be challenged by the healthcare services tradition and regulation for using diagnosis has been shown in a study of adolescents’ perspectives on shared decision-making in mental healthcare [ 55 ]. A practice perceived as labeling by the adolescents, indicating that steps towards a common understanding of mental health needs to be taken from several directions [ 55 ]. In a constructionist investigation to distinguish everyday challenges from mental health problems, instead of asking the question, “What is mental health?” we should perhaps ask, “How is the word ‘mental health’ used, and in what context and type of mental health episode?” [ 26 ]. This is an area for future studies to explore.

Methodological considerations

The first limitation we want to acknowledge, as for any scoping review, is that the results are limited by the search terms included in the database searches. However, by conducting the searches with the help of an experienced librarian we have taken precautions to make the searches as inclusive as possible. The second limitation concerns the lack of homogeneous, or any results at all, according to different age groups, gender, socioeconomic status, and year when the study was conducted. It is well understood that age is a significant determinant in an individual’s conceptualization of more abstract phenomena such as mental health. Some of the studies approached only one age group but most included a wide age range, making it difficult to say anything specific about a particular age. Similar concerns are valid for gender. Regarding socioeconomic status, only one study reported this as a finding. However, this could be an outcome of the choice of methods we had — i.e., qualitative methods, where the aim seldom is to investigate differences between groups and the sample is often supposed to be a variety. It could also depend on the relatively small number of participants that are often used in focus groups of individual interviews- there are not enough participants to compare groups based on gender or socioeconomic status. Finally, we chose studies from countries that could be viewed as having similar development and perspective on mental health among adolescents. Despite this, cultural differences likely account for many youths’ conceptualizations of mental health. According to Meldahl et al. [ 56 ], adolescents’ perspectives on mental health are affected by a range of factors related to cultural identity, such as ethnicity, race, peer and family influence, religious and political views, for example. We would also like to add organizational cultures, such as the culture of the school and how schools work with mental health and related concepts [ 56 ].

Conclusions and implications

Based on our results, we argue that there is a need to establish a common language for discussing mental health. This common language would enable better communication between adults and children and youth, ensuring that the content of the words used to describe mental health is unambiguous and clear. In this endeavor, it is essential to actively listen to the voices of children and youth, as their perspectives will provide us with clearer understanding of the experiences of being young in today’s world. Another way to develop a common language around mental health is through mental health education. A common language based on children’s and youth’s perspectives can guide school personnel, professionals, and parents when discussing and planning health interventions and mental health education. Achieving a common understanding through mental health education of adults and youth could also help clarify the boundaries between everyday challenges and problems needing treatment. It is further important to raise awareness of the positive aspect of mental health—that is, knowledge of what makes us flourish mentally should be more clearly emphasized in teaching our children and youth about life. It should also be emphasized in competence development for school personnel so that we can incorporate knowledge about mental well-being in everyday meetings with children and youth. In that way, we could help children and youth develop knowledge that mental health could be improved or at least maintained and not a static condition.

Key questions: research priorities for student mental health

The high prevalence of mental distress among university students is gaining academic, policy and public attention. As the volume of research into student mental health increases, it is important to involve students to ensure that the evidence produced can translate into meaningful improvements.

For the first time, we consult UK students about their research priorities on student mental health.

This priority setting exercise involved current UK university students who were asked to submit three research questions relating to student mental health. Responses were aggregated into themes through content analysis and considered in the context of existing research. Students were involved throughout the project, including inception, design, recruitment, analysis and dissemination.

UK university students ( N = 385) submitted 991 questions, categorised into seven themes: epidemiology, causes and risk factors, academic factors and work–life balance, sense of belonging, intervention and services, mental health literacy and consequences. Across themes, respondents highlighted the importance of understanding the experience of minority groups.

Conclusions

Students are interested in understanding the causes and consequences of poor mental health at university, across academic and social domains. They would like to improve staff and students’ knowledge about mental health, and have access to evidence-based support. Future research should take a broad lens to evaluate interventions; considering how services are designed and delivered, and investigating institutional and behavioural barriers to accessibility, including how this varies across different groups within the student population.

In the context of increasing prevalence of youth and young adult mental health problems, 1 , 2 including university students, 3 concern about mental health in the university setting is mounting and gaining media and public attention. 4 Increasing demand for services on campus has been observed internationally. 2 , 3 However, current approaches lack a solid evidence base, 5 , 6 and students have voiced concerns that existing services do not meet their needs. 7 In the UK, representatives of university leadership and students are urging the sector to adopt a whole-institution approach. 8 , 9 However, questions about how to achieve this remain unanswered. Eliciting student perspectives and experiences has been highlighted as an enabling strategy for the sector to develop effective and targeted initiatives attuned to diverse student needs and situated within a whole-university approach. 9 As research efforts mount, 10 it is important to involve students to ensure that work in this field translates into meaningful improvements attuned to students’ lived experiences. 11 This project set out to consult students in the UK on their priorities for future research into student mental health. Our aim is to ensure that the student voice is influential in shaping the direction of future research.

Lived experience involvement

The project was initiated through the UK Research and Innovation funded Student Mental Health Research Network (SMaRteN), with a steering group developed from the SMaRteN leadership team. The group recruited diverse stakeholders, including students (both with and without lived experience of mental health difficulties at university), clinical psychologists, tutors and academic researchers. Co-creation was central to this project. This is distinguished from student consultation and participation, by the active involvement of students as equal stakeholders, 12 reciprocally sharing knowledge and networks as part of a strengths and asset-based approach. 13 Students were operating in a ‘peer researcher’ context, and worked with academic researchers to design the methodology, recruit a diverse student sample, analyse data and write up the findings. Several student peer researchers are authors on this paper.

Participants

All procedures contributing to this work comply with the ethical standards of the relevant national and institutional committees on human experimentation and with the Helsinki Declaration of 1975, as revised in 2008. All procedures involving human participants were approved by the university ethics board (approval number LRS-19/20-14288). All participants were provided with information about the study and the opportunity to contact the researchers to ask questions before providing informed consent through an online form.

Our sample included 385 UK university students, who responded to advertisements publicised by SMaRteN and the student mental health charity, Student Minds. Advertisements were circulated through newsletters, Facebook, Twitter and Instagram. We did not provide any monetary incentive for participation. Adverts and the study information sheet reminded participants that the survey would provide an opportunity to help shape the future of research in this area, with SMaRteN funding being allocated to address the top priority questions.

Our sample was primarily under 25 years old ( n  = 285, 74%) and mostly comprised women ( n  = 251, 65%). Our sample included UK domiciled ( n  = 279, 72%) and international ( n  = 106, 28%) students and was 72% White, 15% Asian and 6% Black. A substantial minority of respondents identified as a sexual or romantic minority ( n  = 118, 31%) and/or reported having a disability ( n  = 95, 25%). Students represented all years and levels of study (undergraduate, 66%; taught postgraduate, 17%; postgraduate research, 15%), and represented most subject areas (see Table 1 ).

Data collection was carried out via an online survey hosted on Qualtrics (Seattle, WA, USA; see https://www.qualtrics.com/uk/ ) between October 2019 and February 2020. The survey was designed to be as short and simple as possible to make participation as easy. Respondents submitted up to three questions in response to the prompt: ‘In terms of student mental health, what do you think are the priority issues for researchers to explore?’. There was no word limit for the respondents’ submissions. After submitting questions, respondents were asked to complete demographic details.

Data analysis

The objective of our analysis was to understand respondents’ recommendations for future research and categorise these to create a shortlist of research priorities. We sought to capture student recommendations without passing judgement regarding the value of the research topic or whether the question had already been addressed.

A team of 26 students were involved in analysing the data, supported by experienced researchers. The student team were recruited through SMaRteN from universities across the UK. Selection focused on bringing together a diverse team. SMaRteN hosted a 2-day workshop, covering expenses to bring students together for training and co-creation activities.

To facilitate reflexivity, 14 we followed an iterative approach, with themes developed and refined through consultation with all members of the team. This improves the reliability of analysis, minimising biases arising from individual researchers’ preconceptions. As respondents were invited to provide single sentence questions, without explaining their rationale, it was important not to overanalyse the data. In this context, content analysis was appropriate. As summarised in Fig. 1 , we followed four steps of content analysis, embedding the principles of co-creation in each step. 15

In stage 1, codes were generated inductively, working with the questions provided rather than bringing in any preconceived ideas of the research questions that might be important. This approach was adopted to ensure we think carefully about the questions students were asking, as opposed to trying to fit their questions into the existing research framework. This process was completed independently by members of the steering group, before in-depth discussion, through which a single list of codes was agreed. 14

At stage 2, students checked that all aspects of the content had been covered by revisiting the original questions, determining what should be included and excluded, and developing more detailed codes. 15 For example, although the initial list of codes had included ‘academic pressure’, student analysis here clarified that this should include all questions related academic grades, success, workload, deadline and course-specific challenges.

In stage 3, the lead researcher (N.C.B.) worked iteratively with small groups of students to create categories around the codes, with the goal of reducing the categories without losing the content of units. 16 Returning to the example of academic pressure, we identified parallels between the questions that had been grouped into this code and questions relating to the university extenuating/mitigating circumstances process. Students agreed it was hard to consider the impact of extenuating circumstances without considering these in the wider context of academic culture and assessment practice. Further, most questions relating to work–life balance focused on managing workload, and hence had clear relationship to the questions grouped under academic pressure. As such, we reduced the number of categories by grouping questions together into the category of ‘academic factors’. This process was continuously appraised to ensure categories were internally homogenous and externally heterogenous. 17 For instance, although the questions around academic pressure and extenuating circumstances align, questions relating to academics’ appreciation for the pressure students experienced aligned more clearly with other questions about mental health literacy and academics’ understanding of mental health. In the final stage, categories were checked, named and described.

Across the analysis, although we primarily followed a manifest analysis, describing respondents’ questions as they were presented, at times a more latent approach was necessary to interpret questions that were phrased less clearly. 13 For example, the question ‘What is the effect of workload on students’ mental health?’ can be simply described as asking about student workload, and thus grouped with other questions around workload and academic pressure. In contrast, we received a question reading ‘the amount of work?’. We chose to retain this question and place it within the category of ‘academic factors and work–life balance’. However, here we have assumed the respondent is referring to the work students have to do, rather than the amount of work universities might have to put in to improve student mental health.

In total, 991 questions were submitted and arranged into seven categories. In Table 2 , these categories are set out in descending order of frequency based on the number of questions asked in that category. We discuss each of these categories below.

Summary of key priorities in context of existing research

Intervention and services

The efficacy of existing services (including counselling, workshops and drop-in services) was raised, including whether these services meet the needs of a diverse student population. Respondents suggested the potential effects of a broad range of specific and sometimes novel interventions, including physical activity, yoga, mindfulness, social activities and events, and sleeping pods on campus. Questions considered cost-efficacy as well as how to increase funding.

Respondents questioned the appropriate balance between preventative work and responsive treatment, and how university support services should be designed to meet needs ranging from well-being through to complex and enduring mental health problems: ‘How can the support for student well-being versus chronic/severe mental illness be differentiated and acknowledged as separate issues?’

Respondents identified a need to clarify where the boundaries of responsibility between the National Health Service (NHS) and university services should lie and how these services should be better integrated, especially with the split between home and term-time addresses: ‘What is the role of universities in treating, preventing, helping with mental health? Where do they fit in with the NHS, charities and family/social structures?’ Questions asked whether there is adequate provision of professional mental health support for students, whether this is suitably accessible and what steps can be taken to improve availability and accessibility.

Academic factors and work–life balance

Respondents queried how academic pressure, including challenging content, high workload and a pressure to succeed, contribute to mental health problems. This pressure also included how academic success affects self-worth and how to overcome feelings of shame or embarrassment when struggling academically. Pressure was raised in relation to postgraduate students, with a focus on the relationship between mental health, performance and output. Respondents asked what steps can be taken to help those studying at university to cope with pressure: ‘How can students’ resilience and coping be increased so they are best equipped to deal with HE [higher education] study?’

Participants questions indicated that methods of assessment at university may affect mental health and asked whether changes to assessment design could reduce negative effects. Respondents were interested in examinations versus coursework, as well as how deadlines affect stress. A few questions considered the accessibility and efficacy of university extenuating circumstances: ‘Are universities able/willing to make the more flexible adjustments needed for students with long-term mental health conditions to engage?’

University teaching, including module organisation and structure, number of contact hours and online versus in-person teaching, were raised as potentially affecting mental health. Teaching style changes between school and university were also flagged as possibly problematic: ‘I feel like a lot of people are struggling with the first year. How can we make the gap between uni and high school smaller?’ These questions were raised by students across academic disciplines. Healthcare students uniquely also questioned how placements affect mental health.

Respondents asked about the challenge of time management and maintaining balance in their lives. Questions considered how to balance academic work with a social life and part-time job, and postulated whether trying to achieve this places strain on relationships and well-being. Although there were comparatively few questions relating to balance, students involved in the analysis requested that this theme be highlighted because of its relevance and importance.

Mental health literacy

Questions included whether, and in what ways, a culture of increased awareness, education and conversation would affect student mental health: ‘How is the growing awareness of mental health impacting student's mental health?’ Students were concerned about to identify mental health problems in themselves and their peers, and asked for more knowledge about how to respond to and help someone struggling with mental health problems. The importance of providing support to those who are helping friends with mental health problems was also highlighted. Students wanted knowledge of self-help strategies, and questioned how best to manage and cope with their own mental health problems at university: ‘What steps can students take to minimise their risk of adverse mental health issues?’ This theme also included whether students know what support and advice is available at university, and how they can access it, including how comfortable people feel reaching out for support, the role of stigma and shame, and how to encourage help-seeking behaviour.

Academic staff also play a part in creating a culture around mental health, and so respondents were interested in their mental health literacy and suggested providing resources, training programmes or policy implementation to help staff recognise and support students with mental health problems. Some questions considered whether students feel they are treated as individuals or in a more depersonalised and anonymous manner, and what impact this has on student mental health: ‘Would students suffering with poor mental health be able to work better with more consideration from teachers?’

Causes and risk factors

Identifying potential risk and protective factors for poor mental health was highlighted: ‘Which students are most at risk of poor mental health/well-being and why? And most likely to have good mental health and why – protective factors?’ Respondents posed questions about underlying reasons, triggers or drivers for problems, with some assuming that university has a negative impact on mental health: ‘What is causing mental illness at university, and is it a systemic problem?’ Specific possible contributing factors included student finances, living arrangements, drug and alcohol use, unhealthy lifestyles and concerns for future career prospects. Questions about living arrangements considered the impact of living away from home, transitioning between home and term-time addresses, communal versus solitary living and how living in halls of residence affects mental health. Respondents queried how a sense of belonging and academic factors, including the challenge of finding a work–life balance, might contribute to mental health problems. These questions have not been included here because there was sufficient interest to create independent categories.

Sense of belonging

Respondents wanted to know whether all students feel valued, included and appreciated within their university community, and how to improve this: ‘How can students feel more ‘at home’ and comfortable in their universities?’ Loneliness and isolation were raised, particularly the reasons why students are lonely, how this affects mental health and what can be done to reduce it. Respondents questioned how to make meaningful connections, and why students may feel alone despite being surrounded by people. Respondents considered how student social life affects mental health, including the role of societies and sports groups as well as negative experiences such as peer pressure, elitism and bullying.

Questions considered these problems from the perspective of minority or vulnerable groups, with issues surrounding loneliness being raised specifically for international, mature and commuter students. Respondents queried whether the university environment is inclusive for neurodiverse, minority ethnic, LGBTQ+, working class and disabled students, and how a lack of representation may accentuate loneliness. Victimisation and discrimination, including racism and sexual assault, were identified as potentially contributing to mental health problems at university: ‘How safe do you feel on your campus? Specifically, relevant for minoritized groups, i.e. BAME, LGBTQ, non-neurotypical students, etc. and women’.

Epidemiology

Questions falling into this category considered the prevalence of mental health problems among university students, including identifying the most common conditions, how the incidence of these problems is changing in universities and how prevalence differs between students and non-students. Many questions revealed underlying presumptions that student mental health is declining, and that students are more vulnerable to mental health problems than their peers: ‘Why has the prevalence of mental health problems in university students increased?’ They also questioned when mental health problems develop, whether this is before or after coming to university and how the move to university changes peoples’ experiences.

Consequences

A small number of questions asked about the consequences of mental health problems at university, and particularly the impact on academic achievement and social life. Respondents asked about drop-out rates in relation to mental health, and consequences for career development. Respondents were interested in the prognosis for those who struggle with mental health problems at university, including rates of recovery.

The aim of this co-creation project was to identify the mental health research priorities of university students and enable the student voice to shape the direction of future research. Our study identified seven key areas for future research. Many themes overlapped, reflecting the interconnectedness of different facets of student life. As summarised in Table 2 , we have positioned the students’ priorities in the context of the existing research, which is often small scale and narrowly focused, with limited consideration of racial, ethnic and sexual minorities. The project was undertaken before the COVID-19 pandemic, which has resulted in substantial disruption to students’ lives and rapid changes to university practices, and highlights the long-term challenges facing student mental health. It is important that student priorities are considered as the higher education sector transitions to a post-pandemic world.

Although the data available suggest that short-term embedded counselling at university is clinically effective, 18 evaluation of the efficacy of university mental health services has been minimal. 19 There has been limited evaluation of interventions and services as part of a whole-university approach, 5 , 6 and to our knowledge, no published evaluation of the impact of collaboration between universities and the NHS. Although there has been some consideration of non-clinical interventions such as yoga and exercise, most studies are of poor quality, and it is not possible to rank which interventions work best, where and for whom. 6 Future studies must take a broader lens to evaluate interventions for students, especially how they are designed, delivered and made accessible, and should employ robust evaluation of service efficacy. In line with student priorities, it is vital that future research considers the efficacy of services for the diverse student population.

International research indicates that student workload is a major factor contributing to stress and can result in prolonged study times or drop-out. 20 , 21 However, despite growing research interest in a ‘whole of curriculum approach’, knowledge about how to support student mental health through curricula and pedagogy is lacking. Preliminary evidence from the USA demonstrated that a multidimensional curricula intervention involving reduction in contact time, a change in grading system, collaborative and practical learning initiatives, and an embedded resilience and mindfulness intervention, resulted in significant decreases in depressive and anxiety symptoms among medical students, with corresponding increases in quality of life, group cohesion, student satisfaction and examination scores. 22 This suggests that there is a promising way forward that could be adopted in the UK context across different types of degrees, in keeping with the many student questions on this topic.

Mental health literacy is defined as ‘knowledge and belief about mental disorders which aid their recognition, management or prevention’. 23 Within this, understanding how to look after your own mental health and support peers is fundamental. 24 Preliminary research has demonstrated potential efficacy and acceptability of peer support programmes 25 and programmes to improve student mental health literacy among students. 26 However, further research is needed to evaluate these more thoroughly and compare different approaches. Academics are under increased pressure to support student mental health, but many find it challenging to understand their role and the best response. 27 Research findings around mental health literacy are varied, with some studies identifying knowledge gaps 28 and others noting good levels of literacy among students and staff. 29 Exploring how staff and students can support themselves and others with mental health difficulties is an important priority for future research.

Research has only focused on whether a specific factor, in isolation, is relevant to mental health. For example, there is strong evidence of relationships between mental health problems and financial stress, 30 , 31 drug and alcohol consumption, 32 , 33 isolation and loneliness, 34 and sleep disruption 35 , 36 among students, as well as experiences of adverse events before and during university. 37 Although studies have increasingly explored the link between factors such as accommodation environments, 38 , 39 and physical activity 40 and student mental health, investigating general and comparative risk, protective and causal factors associated with mental health problems among students remains a high priority.

The repeated use of the word ‘loneliness’ within submitted questions was striking. There are strong links between loneliness and mental health problems, 41 and loneliness is particularly associated with the transition from adolescence to adulthood. 42 Loneliness appears to be accentuated by the significant upheaval in social networks that occurs when young adults leave the family home. 43 Research focusing on loneliness and student and postgraduate mental health is developing, 34 , 44 but studies to establish how student friendship groups form, how and why students experience loneliness at university and how student loneliness can be prevented should continue, particularly with student input. The COVID-19 pandemic caused further disruption to students’ social networks, with public concern for students missing the university experience. 45 , 46 It will be important for research exploring the impact of COVID-19 to recognise that challenges around sense of belonging on the university campus predate the pandemic.

A sense of belonging is unique to the individual. As recognised by the students in our study, it is vital for issue of belonging and loneliness to be investigated among minority groups. Although there is a substantive body of research on attainment gaps for students from minority ethnic backgrounds in UK higher education, 47 there are evidence gaps related to how structural exclusion affects mental health. 48

Existing evidence suggests 20–40% of university students are likely to meet criteria for mental health problems, and prevalence rates have been increasing over recent years. 2 , 3 Analysis of large population data-sets provides conflicting evidence about the relative prevalence of mental health problems between university students and peers not in higher education. 2 , 49 With notable exceptions, 50 , 51 there has been limited work within the UK identifying how mental health problems might vary across years of study, academic disciplines and universities, although this has been explored extensively within the USA. 21 Current data around the prevalence of mental health problems for minority student groups also remains limited.

In line with student priorities, future research must provide more precise estimates of the prevalence of student mental health problems, and identify how these vary across the student population. This will have important implications for service planning and provision. Given many students have pre-existing beliefs regarding prevalence and trends of mental health problems at university, clear communication of existing data and future findings is imperative.

In keeping with students’ concerns, research suggests mental health does affect educational achievement at university. 52 – 54 However longitudinal studies assessing long-term consequences across a wider breadth of domains, including social life and future career development, are lacking.

Strengths and limitations

Student involvement in every stage of the study increased the likelihood that the project would be responsive to students’ needs and research priorities. Our sample was broadly representative of the student population, although it overrepresented women, underrepresented undergraduate students and overrepresented students studying sciences, primarily because of a large representation of students studying medicine and dentistry, biological sciences (including psychology) and computer science (see Table 1 ). Given the widespread underrepresentation of men in research into student mental health, it will remain important for future research to develop specific strategies to consult and engage male students in research design. As a self-selective sample, it is important to recognise that the voices of students who care passionately about student mental health are likely to have been overrepresented in this project.

In conclusion, this project identifies seven key priorities for future research into student mental health from the perspective of UK university students. Students’ questions are mostly unmet in the existing literature, with less research into the mental health of racial, ethnic and sexual minority student communities. Research is needed in each of these seven areas, and Table 2 highlights key questions to be answered. However, three areas stand out as particularly important. ‘Interventions and services’ was the largest category of questions. This is also an area where there are research gaps. We do not need more research evaluating whether one-to-one clinical interventions are effective. Rather, research needs to assess the whole-university approach, understand the range of needs across a diverse student population and consider the broad student experience of services, from initial help-seeking through identifying appropriate support, triaging, waiting lists and using the service. In contrast to the attention students have given to academic factors, the research in this area is sparce. We need robust, large-scale evaluations of the impact of curricula and pedagogy on student mental health. Finally, there is a stark gap between student interest and research exploring sense of belonging. Future research must address the university social experience, to enhance our understanding of how this relates to student mental health and how it might be leveraged to improve mental health.

Our results have important implications for future funding to ensure research produces knowledge that is useful, relevant and meaningful to diverse student populations, as well as ensuring that knowledge can be translated into positive and practical changes within the higher education sector.

Project to Learn about Youth PLAY logo

The Project to Learn About Youth – Mental Health (PLAY-MH) analyzed information collected from four communities. The focus was to study attention-deficit/hyperactivity disorder (ADHD)  and other  externalizing and internalizing  disorders, as well as  tic disorders  in school-aged children. The purpose was to learn more about public health prevention and intervention strategies to support children’s health and development.

Healthcare providers, public health researchers, educators, and policy makers can get information about the prevalence of children’s mental health disorders from a variety of sources. Data sources, such as national surveys, community-based studies, and administrative claims data (like healthcare insurance claims), use different study methods and provide different types of information, each with advantages and disadvantages. Advantages and disadvantages for different data sources include the following:

  • National surveys have large sample sizes that are needed to create estimates at the national and state levels. However, they also generally use a parent’s report of the child’s diagnosis, which means that the healthcare provider has to give an accurate diagnosis and the parent has to accurately remember what it was.
  • Community-based studies offer the opportunity to observe children’s symptoms, which means that even children who have not been diagnosed or do not have the right diagnosis could be found. However, these studies are typically done in small geographic areas, so findings are not necessarily the same in other communities.
  • Administrative claims are typically very large datasets with information on diagnosis and treatment directly from the providers, which allows tracking changes over time. Because they are recorded for billing purposes, diagnoses or services that would not be reimbursed from the specific health insurance might not be recorded in the data.

Using different sources of data together provides more information because it is possible to describe the following:

  • Children with a diagnosed condition compared to children who have the same symptoms, but are not diagnosed
  • Differences between populations with or without health insurance
  • How estimates for mental health disorders change over time

National data on children’s mental health

A comprehensive report from the Centers for Disease Control and Prevention (CDC), Mental Health Surveillance Among Children — United States, 2013 – 2019 , described federal efforts on monitoring mental disorders, and presented estimates of the number of children with specific mental disorders as well as for positive indicators of mental health. The report was developed in collaboration with the Substance Abuse and Mental Health Services Administration (SAMHSA ), the National Institute of Mental Health (NIMH ), and the Health Resources and Services Administration (HRSA ). It represents an update to the first ever cross-agency children’s mental health surveillance report in 2013.

The goal is now to build on the strengths of federal agencies serving children with mental disorders to:

  • Develop better ways to document how many children have these disorders,

This report is an important step on the road to recognizing the impact of childhood mental disorders and developing a public health approach to address children’s mental health.

Mental health includes emotional, psychological, and social well-being. It is more than the absence of a mental illness—it’s essential to your overall health and quality of life. Self-care can play a role in maintaining your mental health and help support your treatment and recovery if you have a mental illness.

How can I take care of my mental health?

Self-care means taking the time to do things that help you live well and improve both your physical health and mental health. This can help you manage stress, lower your risk of illness, and increase your energy. Even small acts of self-care in your daily life can have a big impact.

Here are some self-care tips:

  • Get regular exercise.  Just 30 minutes of walking every day can boost your mood and improve your health. Small amounts of exercise add up, so don’t be discouraged if you can’t do 30 minutes at one time.
  • Eat healthy, regular meals and stay hydrated.  A balanced diet and plenty of water can improve your energy and focus throughout the day. Pay attention to your intake of caffeine and alcohol and how they affect your mood and well-being—for some, decreasing caffeine and alcohol consumption can be helpful.
  • Make sleep a priority . Stick to a schedule, and make sure you’re getting enough sleep. Blue light from devices and screens can make it harder to fall asleep, so reduce blue light exposure from your phone or computer before bedtime.
  • Try a relaxing activity.  Explore relaxation or wellness programs or apps, which may incorporate meditation, muscle relaxation, or breathing exercises. Schedule regular times for these and other healthy activities you enjoy, such as listening to music, reading, spending time in nature, and engaging in low-stress hobbies.
  • Set goals and priorities.  Decide what must get done now and what can wait. Learn to say “no” to new tasks if you start to feel like you’re taking on too much. Try to appreciate what you have accomplished at the end of the day.
  • Practice gratitude.  Remind yourself daily of things you are grateful for. Be specific. Write them down or replay them in your mind.
  • Focus on positivity . Identify and challenge your negative and unhelpful thoughts.
  • Stay connected.  Reach out to friends or family members who can provide emotional support and practical help.

Self-care looks different for everyone, and it is important to find what you need and enjoy. It may take trial and error to discover what works best for you.

When should I seek professional help?

Seek professional help if you are experiencing severe or distressing symptoms that have lasted 2 weeks or more, such as:

  • Difficulty sleeping
  • Changes in appetite or unplanned weight changes
  • Difficulty getting out of bed in the morning because of mood
  • Difficulty concentrating
  • Loss of interest in things you usually find enjoyable
  • Inability to complete usual tasks and activities
  • Feelings of irritability, frustration, or restlessness

They can refer you to a qualified mental health professional, such as a psychologist, psychiatrist, or clinical social worker, who can help you figure out the next steps. In CDC survey, 37% of U.S. high school students report regular mental health struggles during COVID-19 pandemic

Many high school students have reported experiencing mental health challenges during the coronavirus outbreak, according to recently published survey findings from the Centers for Disease Control and Prevention (CDC). High school students who are gay, lesbian or bisexual, as well as girls, were especially likely to say their mental health has suffered during the pandemic.

This analysis explores U.S. high school students’ self-reported mental health challenges during the COVID-19 pandemic. It expands on Pew Research Center surveys that have explored U.S. adults’ mental health difficulties during this time. Not all of the survey questions asked specifically about mental health during the pandemic.

This analysis relies on the Center for Disease Control and Prevention’s Adolescent Behaviors and Experiences Survey (ABES), which was conducted from January to June 2021 to assess students’ health-related behaviors and experiences during the COVID-19 pandemic. ABES surveyed high school students in grades 9-12 attending U.S. public and private schools. More information about the survey and its methodology can be found on the CDC’s website.

The results from this one-time survey are not directly comparable to previous CDC surveys on these topics.

Overall, 37% of students at public and private high schools reported that their mental health was not good most or all of the time during the pandemic, according to the CDC’s Adolescent Behaviors and Experiences Survey , which was fielded from January to June 2021. In the survey, “poor mental health” includes stress, anxiety and depression. About three-in-ten high school students (31%) said they experienced poor mental health most or all of the time in the 30 days before the survey. In addition, 44% said that, in the previous 12 months, they felt sad or hopeless almost every day for at least two weeks in a row such that they stopped doing some usual activities. (Not all of the survey questions asked specifically about mental health during the pandemic.)

High school students who are gay, lesbian or bisexual reported higher rates of mental health stresses than their heterosexual (straight) peers. The share of LGB high schoolers who said their mental health was not good most of the time or always during the pandemic was more than double that of heterosexual students (64% vs. 30%). More than half of LGB students (55%) said they experienced poor mental health at least most of the time in the 30 days before the survey, while 26% of heterosexual teens said the same. And about three-quarters of LGB high schoolers (76%) said they felt sad or hopeless almost daily for at least two weeks such that they stopped doing some of their usual activities, compared with 37% of heterosexual students.

There were also differences by gender. About half of high school girls (49%) said their mental health was not good most of the time or always during the COVID-19 outbreak – roughly double the share of boys who said this (24%). And roughly four-in-ten girls (42%) reported feeling this way in the 30 days before the survey; 20% of boys said the same. About six-in-ten high school girls (57%) reported that at some point in the 12 months before taking the survey (in the first half of 2021) they felt sad or hopeless almost every day for at least two weeks in a row such that they stopped doing some usual activities, compared with 31% of high school boys who said this.

LGB high schoolers were also more likely than their heterosexual peers to have sought mental health care – including treatment or counseling for alcohol or drug use – via telemedicine during the COVID-19 pandemic. Around one-in-five LGB students (19%) said they received treatment this way at some point during the pandemic, compared with 6% of heterosexual students. Girls were more likely than boys to have received mental health care through telemedicine (10% vs. 7%, respectively).

Pandemic-related disruptions to schooling, socializing and family life have created a situation that the U.S. surgeon general has described as a “ youth mental health crisis ,” with high rates of teens experiencing distress. But public health experts had called attention to teen mental health even before the coronavirus outbreak. For instance, a separate CDC survey conducted in 2015 found that LGB teens were at greater risk of depression than their heterosexual peers. And a Pew Research Center analysis of pre-pandemic data from the National Survey for Drug Use and Health showed teenage girls were more likely than their male peers to report recent experiences with depression , as well as to receive treatment for it.
