Everything you need to know to write a high-quality research question

If you’ve landed on this page, you’re probably asking yourself, “ What is a research question? ”. Well, you’ve come to the right place. In this post, we’ll explain what a research question is , how it’s differen t from a research aim, and how to craft a high-quality research question that sets you up for success.

As the name suggests, the research question is the core question (or set of questions) that your study will (attempt to) answer .

In many ways, a research question is akin to a target in archery . Without a clear target, you won’t know where to concentrate your efforts and focus. Essentially, your research question acts as the guiding light throughout your project and informs every choice you make along the way.

Let’s look at some examples:

What impact does social media usage have on the mental health of teenagers in New York?
How does the introduction of a minimum wage affect employment levels in small businesses in outer London?
How does the portrayal of women in 19th-century American literature reflect the societal attitudes of the time?
What are the long-term effects of intermittent fasting on heart health in adults?

As you can see in these examples, research questions are clear, specific questions that can be feasibly answered within a study. Research Questions vs Research Aims

At this point, you might be asking yourself, “ How is a research question different from a research aim? ”. Within any given study, the research aim and research question (or questions) are tightly intertwined , but they are separate things . Let’s unpack that a little.

A research aim is typically broader in nature and outlines what you hope to achieve with your research. It doesn’t ask a specific question but rather gives a summary of what you intend to explore.

The research question, on the other hand, is much more focused . It’s the specific query you’re setting out to answer. It narrows down the research aim into a detailed, researchable question that will guide your study’s methods and analysis.

Let’s look at an example:

Research Aim: To explore the effects of climate change on marine life in Southern Africa.
Research Question: How does ocean acidification caused by climate change affect the reproduction rates of coral reefs?

As you can see, the research aim gives you a general focus , while the research question details exactly what you want to find out.

Types of research questions

Now that we’ve defined what a research question is, let’s look at the different types of research questions that you might come across. Broadly speaking, there are (at least) four different types of research questions – descriptive , comparative , relational , and explanatory . 

Descriptive questions ask what is happening. In other words, they seek to describe a phenomena or situation . An example of a descriptive research question could be something like “What types of exercise do high-performing UK executives engage in?”. This would likely be a bit too basic to form an interesting study, but as you can see, the research question is just focused on the what – in other words, it just describes the situation.

Comparative research questions , on the other hand, look to understand the way in which two or more things differ , or how they’re similar. An example of a comparative research question might be something like “How do exercise preferences vary between middle-aged men across three American cities?”. As you can see, this question seeks to compare the differences (or similarities) in behaviour between different groups.

Next up, we’ve got exploratory research questions , which ask why or how is something happening. While the other types of questions we looked at focused on the what, exploratory research questions are interested in the why and how . As an example, an exploratory research question might ask something like “Why have bee populations declined in Germany over the last 5 years?”. As you can, this question is aimed squarely at the why, rather than the what.

Last but not least, we have relational research questions . As the name suggests, these types of research questions seek to explore the relationships between variables . Here, an example could be something like “What is the relationship between X and Y” or “Does A have an impact on B”. As you can see, these types of research questions are interested in understanding how constructs or variables are connected , and perhaps, whether one thing causes another.

Of course, depending on how fine-grained you want to get, you can argue that there are many more types of research questions , but these four categories give you a broad idea of the different flavours that exist out there. It’s also worth pointing out that a research question doesn’t need to fit perfectly into one category – in many cases, a research question might overlap into more than just one category and that’s okay.

The key takeaway here is that research questions can take many different forms , and it’s useful to understand the nature of your research question so that you can align your research methodology accordingly.

How To Write A Research Question

As we alluded earlier, a well-crafted research question needs to possess very specific attributes, including focus , clarity and feasibility . But that’s not all – a rock-solid research question also needs to be rooted and aligned . Let’s look at each of these.

A strong research question typically has a single focus. So, don’t try to cram multiple questions into one research question; rather split them up into separate questions (or even subquestions), each with their own specific focus. As a rule of thumb, narrow beats broad when it comes to research questions.

Clear and specific

A good research question is clear and specific, not vague and broad. State clearly exactly what you want to find out so that any reader can quickly understand what you’re looking to achieve with your study. Along the same vein, try to avoid using bulky language and jargon – aim for clarity.

Unfortunately, even a super tantalising and thought-provoking research question has little value if you cannot feasibly answer it. So, think about the methodological implications of your research question while you’re crafting it. Most importantly, make sure that you know exactly what data you’ll need (primary or secondary) and how you’ll analyse that data.

A good research question (and a research topic, more broadly) should be rooted in a clear research gap and research problem . Without a well-defined research gap, you risk wasting your effort pursuing a question that’s already been adequately answered (and agreed upon) by the research community. A well-argued research gap lays at the heart of a valuable study, so make sure you have your gap clearly articulated and that your research question directly links to it.

As we mentioned earlier, your research aim and research question are (or at least, should be) tightly linked. So, make sure that your research question (or set of questions) aligns with your research aim . If not, you’ll need to revise one of the two to achieve this.

Typically, a research question is phrased as a question, not a statement. A question clearly indicates what you’re setting out to discover.

Can a research question be too broad or too narrow?

Yes. A question that’s too broad makes your research unfocused, while a question that’s too narrow limits the scope of your study.

Here’s an example of a research question that’s too broad:

“Why is mental health important?”

Conversely, here’s an example of a research question that’s likely too narrow:

“What is the impact of sleep deprivation on the exam scores of 19-year-old males in London studying maths at The Open University?”

Can I change my research question during the research process?

A good research question is focused, specific, practical, rooted in a research gap, and aligned with the research aim. If your question meets these criteria, it’s likely a strong question.

Is a research question similar to a hypothesis?

Not quite. A hypothesis is a testable statement that predicts an outcome, while a research question is a query that you’re trying to answer through your study. Naturally, there can be linkages between a study’s research questions and hypothesis, but they serve different functions.

How are research questions and research objectives related?

The research question is a focused and specific query that your study aims to answer. It’s the central issue you’re investigating. The research objective, on the other hand, outlines the steps you’ll take to answer your research question. Research objectives are often more action-oriented and can be broken down into smaller tasks that guide your research process. How to Write a Good Research Question (w/ Examples)

What is a Research Question?

A research question is the main question that your study sought or is seeking to answer. A clear research question guides your research paper or thesis and states exactly what you want to find out, giving your work a focus and objective. Learning  how to write a hypothesis or research question is the start to composing any thesis, dissertation, or research paper. It is also one of the most important sections of a research proposal . 

A good research question not only clarifies the writing in your study; it provides your readers with a clear focus and facilitates their understanding of your research topic, as well as outlining your study’s objectives. Before drafting the paper and receiving research paper editing (and usually before performing your study), you should write a concise statement of what this study intends to accomplish or reveal.

Research Question Writing Tips

Listed below are the important characteristics of a good research question:

A good research question should:

  • Be clear and provide specific information so readers can easily understand the purpose.
  • Be focused in its scope and narrow enough to be addressed in the space allowed by your paper
  • Be relevant and concise and express your main ideas in as few words as possible, like a hypothesis.
  • Be precise and complex enough that it does not simply answer a closed “yes or no” question, but requires an analysis of arguments and literature prior to its being considered acceptable. 
  • Be arguable or testable so that answers to the research question are open to scrutiny and specific questions and counterarguments.

Some of these characteristics might be difficult to understand in the form of a list. Let’s go into more detail about what a research question must do and look at some examples of research questions.

The research question should be specific and focused 

Research questions that are too broad are not suitable to be addressed in a single study. One reason for this can be if there are many factors or variables to consider. In addition, a sample data set that is too large or an experimental timeline that is too long may suggest that the research question is not focused enough.

A specific research question means that the collective data and observations come together to either confirm or deny the chosen hypothesis in a clear manner. If a research question is too vague, then the data might end up creating an alternate research problem or hypothesis that you haven’t addressed in your Introduction section .

The research question should be based on the literature 

An effective research question should be answerable and verifiable based on prior research because an effective scientific study must be placed in the context of a wider academic consensus. This means that conspiracy or fringe theories are not good research paper topics.

Instead, a good research question must extend, examine, and verify the context of your research field. It should fit naturally within the literature and be searchable by other research authors.

References to the literature can be in different citation styles and must be properly formatted according to the guidelines set forth by the publishing journal, university, or academic institution. This includes in-text citations as well as the Reference section . 

The research question should be realistic in time, scope, and budget

There are two main constraints to the research process: timeframe and budget.

A proper research question will include study or experimental procedures that can be executed within a feasible time frame, typically by a graduate doctoral or master’s student or lab technician. Research that requires future technology, expensive resources, or follow-up procedures is problematic.

A researcher’s budget is also a major constraint to performing timely research. Research at many large universities or institutions is publicly funded and is thus accountable to funding restrictions. 

The research question should be in-depth

Research papers, dissertations and theses , and academic journal articles are usually dozens if not hundreds of pages in length.

A good research question or thesis statement must be sufficiently complex to warrant such a length, as it must stand up to the scrutiny of peer review and be reproducible by other scientists and researchers.

Research Question Types

Qualitative and quantitative research are the two major types of research, and it is essential to develop research questions for each type of study. 

Quantitative Research Questions

Quantitative research questions are specific. A typical research question involves the population to be studied, dependent and independent variables, and the research design.

In addition, quantitative research questions connect the research question and the research design. In addition, it is not possible to answer these questions definitively with a “yes” or “no” response. For example, scientific fields such as biology, physics, and chemistry often deal with “states,” in which different quantities, amounts, or velocities drastically alter the relevance of the research.

As a consequence, quantitative research questions do not contain qualitative, categorical, or ordinal qualifiers such as “is,” “are,” “does,” or “does not.”

In quantitative research, research questions have the potential to relate to broad research areas as well as more specific areas of study. Qualitative research questions are less directional, more flexible, and adaptable compared with their quantitative counterparts. Thus, studies based on these questions tend to focus on “discovering,” “explaining,” “elucidating,” and “exploring.”

Good and Bad Research Question Examples

Below are some good (and not-so-good) examples of research questions that researchers can use to guide them in crafting their own research questions.

Research Question Example 1

The first research question is too vague in both its independent and dependent variables. There is no specific information on what “exposure” means. Does this refer to comments, likes, engagement, or just how much time is spent on the social media platform?

Second, there is no useful information on what exactly “affected” means. Does the subject’s behavior change in some measurable way? Or does this term refer to another factor such as the user’s emotions?

Research Question Example 2

In this research question, the first example is too simple and not sufficiently complex, making it difficult to assess whether the study answered the question. The author could really only answer this question with a simple “yes” or “no.” Further, the presence of data would not help answer this question more deeply, which is a sure sign of a poorly constructed research topic.

The second research question is specific, complex, and empirically verifiable. One can measure program effectiveness based on metrics such as attendance or grades. Further, “bullying” is made into an empirical, quantitative measurement in the form of recorded disciplinary actions.

Steps for Writing a Research Question

Good research questions are relevant, focused, and meaningful. It can be difficult to come up with a good research question, but there are a few steps you can follow to make it a bit easier.

1. Start with an interesting and relevant topic

Choose a research topic that is interesting but also relevant and aligned with your own country’s culture or your university’s capabilities. Popular academic topics include healthcare and medical-related research. However, if you are attending an engineering school or humanities program, you should obviously choose a research question that pertains to your specific study and major.

2. Do preliminary research  

You can begin doing preliminary research once you have chosen a research topic. Two objectives should be accomplished during this first phase of research. First, you should undertake a preliminary review of related literature to discover issues that scholars and peers are currently discussing. With this method, you show that you are informed about the latest developments in the field.

Secondly, identify knowledge gaps or limitations in your topic by conducting a preliminary literature review . It is possible to later use these gaps to focus your research question after a certain amount of fine-tuning.

3. Narrow your research to determine specific research questions

You can focus on a more specific area of study once you have a good handle on the topic you want to explore. Focusing on recent literature or knowledge gaps is one good option. 

By identifying study limitations in the literature and overlooked areas of study, an author can carve out a good research question. The same is true for choosing research questions that extend or complement existing literature.

4. Evaluate your research question

Make sure you evaluate the research question by asking the following questions:

Is my research question clear?

The resulting data and observations that your study produces should be clear. For quantitative studies, data must be empirical and measurable. For qualitative, the observations should be clearly delineable across categories.

Is my research question focused and specific?

A strong research question should be specific enough that your methodology or testing procedure produces an objective result, not one left to subjective interpretation. Open-ended research questions or those relating to general topics can create ambiguous connections between the results and the aims of the study. 

Is my research question sufficiently complex?

The result of your research should be consequential and substantial (and fall sufficiently within the context of your field) to warrant an academic study. Simply reinforcing or supporting a scientific consensus is superfluous and will likely not be well received by most journal editors.

This chapter discusses (1) the important role of research questions for descriptive, predictive, and causal studies across the three research paradigms (i.e., quantitative, qualitative, and mixed methods); (2) characteristics of quality research questions, and (3) three frameworks to support the development of research questions and their dissemination within scholarly work. For the latter, a description of the P opulation/ P articipants, I ntervention/ I ndependent variable, C omparison, and O utcomes (PICO) framework for quantitative research as well as variations depending on the type of research is provided. Second, we discuss the P articipants, central Ph enomenon, T ime, and S pace (PPhTS) framework for qualitative research. The combination of these frameworks is discussed for mixed-methods research. Further, templates and examples are provided to support the novice health scholar in developing research questions for applied and theoretical studies. Finally, we discuss the Create a Research Space (CARS) model for introducing research questions as part of a research study, to demonstrate how scholars can apply their knowledge when disseminating research.

Developing a research question.

In order to develop a research question, one useful method is to develop “working questions” of all shapes and sizes pertinent to your topic. As you can see below, you can start with a handful of simple working questions that will eventually lead to a viable research question. (Note that these examples also are precursors to the three-tiered, strongest type of thesis, as shown in the revised research questions.)

As you hone your path of inquiry, you may need to zoom in or out in terms of scope. Often, a narrower scope is easier to work with than a broader scope. You will be able to write more and write better if your question calls for more complex thinking.

Consider the diagram above.  As you build a working knowledge of your topic (e.g., as you get a feel for the conversation that began before you arrived at the party), you might complicate or narrow your working questions. Remember to be flexible as you research; you might need to pivot, adjust, re-focus, or replace your research question as you learn more. Consider this imaginary case study as an example of this process:

Jacob began his project by identifying the following areas of interest: racism in the U.S., technology in medicine and health care, and independent film-making. After doing some prewriting and preliminary research on each, he decided he wanted to learn more about racially motivated police violence. He developed working questions:

  • Are police officers likely to make judgments about citizens based on their race?
  • Have police forces instituted policies to avoid racism?
  • Who is most vulnerable to police violence?
  • Who is responsible for overseeing the police?

He realized that he needed to narrow his focus to develop a more viable path of inquiry, eventually ending up with the research question:

  • Over the last 30 years, what populations are most likely to experience police violence in the U.S.?

However, after completing more research, Jacob discovered that his answers came quite readily and consistently: young black men are significantly more vulnerable to become victims of police violence. He realized that he’s not really saying anything new, so he had to tweak his path of inquiry.

Jacob did more freewriting and research to find sources that disagreed with this conclusion or added new layers to his answers. He discovered eventually that there are a handful of police organizations that have made genuine efforts to confront racism in their practices. These groups were working actively against racial violence. He reoriented his research question as follows:

  • Have anti-racist police trainings and strategies been effective in reducing individual or institutional racism over the last 30 years?

Practice: Research Questions and Working Thesis Statements

1. Which of the following is the better research question?

  • How does trash pollute the environment?
  • What is the environmental impact of plastic water bottles?
  • What is the impact of bottled water on the environment?

2. Decide whether or not the following working thesis statements are good or bad:

  • Man has had a major impact on the environment.
  • Marijuana use in Mishawaka, Indiana, has been a problem for law enforcement since the 1970s.
  • Miley Cyrus is a horrible singer.
  • Profilers have played a necessary role in catching serial killers.

a. Bad. This statement is way too vague and broad. What constitutes “major impact”? What aspects of the environment are we talking about? What century are we talking about?

b. Bad. Even if it is true, it is too local and narrow to be supported with national or scholarly research. Sources would probably be limited to local newspaper articles and personal interviews. Can you make those sources “stretch” across a 10 page research paper? Not likely.

c. Bad, because the statement is largely an unfocused opinion. What exactly is “horrible”? How does Miley Cyrus fall into that category? Do you think there are many books or research articles that could support this topic? Probably not.

d. Good. Using this statement for a paper allows you to skip over the sources that do not deal with profiling, that do not deal with the apprehension of serial killers, and that deal only with the injustices of “racial profiling.”  A good working thesis statement saves you time and keeps you focused.

Statistical Research Questions: Five Examples for Quantitative Analysis

How are statistical research questions for quantitative analysis written? This article provides five examples of statistical research questions that will allow statistical analysis to take place.

In quantitative research projects, writing statistical research questions requires a good understanding and the ability to discern the type of data that you will analyze. This knowledge is elemental in framing research questions that shall guide you in identifying the appropriate statistical test to use in your research.

Thus, before writing your statistical research questions and reading the examples in this article, read first the article that enumerates the  four types of measurement scales . Knowing the four types of measurement scales will enable you to appreciate the formulation or structuring of research questions.

Once you feel confident that you can correctly identify the nature of your data, the following examples of statistical research questions will strengthen your understanding. Asking these questions can help you unravel unexpected outcomes or discoveries particularly while doing exploratory data analysis .

Five Examples of Statistical Research Questions

In writing the statistical research questions, I provide a topic that shows the variables of the study, the study description, and a link to the original scientific article to give you a glimpse of the real-world examples.

Topic 1: Physical Fitness and Academic Achievement

A study was conducted to determine the relationship between physical fitness and academic achievement. The subjects of the study include school children in urban schools.

Statistical Research Question No. 1

Is there a significant relationship between physical fitness and academic achievement?

Notice that this study correlated two variables, namely 1) physical fitness, and 2) academic achievement.

To allow statistical analysis to take place, there is a need to define what is physical fitness, as well as academic achievement. The researchers measured physical fitness in terms of  the number of physical fitness tests  that the students passed during their physical education class. It’s simply counting the ‘number of PE tests passed.’

On the other hand, the researchers measured academic achievement in terms of a passing score in Mathematics and English. The variable is the  number of passing scores  in both Mathematics and English.

Both variables are ratio variables. 

Given the statistical research question, the appropriate statistical test can be applied to determine the relationship. A Pearson correlation coefficient test will test the significance and degree of the relationship. But the more sophisticated higher level statistical test can be applied if there is a need to correlate with other variables.

In the particular study mentioned, the researchers used  multivariate logistic regression analyses  to assess the probability of passing the tests, controlling for students’ weight status, ethnicity, gender, grade, and socioeconomic status. For the novice researcher, this requires further study of multivariate (or many variables) statistical tests. You may study it on your own.

Most of what I discuss in the statistics articles I wrote came from self-study. It’s easier to understand concepts now as there are a lot of resource materials available online. Videos and ebooks from places like Youtube, Veoh, The Internet Archives, among others, provide free educational materials. Online education will be the norm of the future. Just maintain an open mind to get rid of your dislike for numbers; that is, if you are one of those who have a hard time understanding mathematical concepts. Topic 2: Climate Conditions and Consumption of Bottled Water

This study attempted to correlate climate conditions with the decision of people in Ecuador to consume bottled water, including the volume consumed. Specifically, the researchers investigated if the increase in average ambient temperature affects the consumption of bottled water.

Statistical Research Question No. 2

Is there a significant relationship between average temperature and amount of bottled water consumed?

In this instance, the variables measured include the  average temperature in the areas studied  and the  volume of water consumed . Temperature is an  interval variable,  while volume is a  ratio variable .

In this example, the variables include the  average temperature  and  volume of bottled water . The first variable (average temperature) is an interval variable, and the latter (volume of water) is a ratio variable.

Now, it’s easy to identify the statistical test to analyze the relationship between the two variables. You may refer to my previous post titled  Parametric Statistics: Four Widely Used Parametric Tests and When to Use Them . Using the figure supplied in that article, the appropriate test to use is, again, Pearson’s Correlation Coefficient.

Topic 3: Nursing Home Staff Size and Number of COVID-19 Cases

An investigation sought to determine if the size of nursing home staff and the number of COVID-19 cases are correlated. Specifically, they looked into the number of unique employees working daily, and the outcomes include weekly counts of confirmed COVID-19 cases among residents and staff and weekly COVID-19 deaths among residents.

Statistical Research Question No. 3

Is there a significant relationship between the number of unique employees working in skilled nursing homes and the following:

  • number of weekly confirmed COVID-19 cases among residents and staff, and
  • number of weekly COVID-19 deaths among residents.

Note that this study on COVID-19 looked into three variables, namely 1) number of unique employees working in skilled nursing homes, 2) number of weekly confirmed cases among residents and staff, and 3) number of weekly COVID-19 deaths among residents.

We call the variable  number of unique employees  the  independent variable , and the other two variables ( number of weekly confirmed cases among residents and staff  and  number of weekly COVID-19 deaths among residents ) as the  dependent variables .

This correlation study determined if the number of staff members in nursing homes influences the number of COVID-19 cases and deaths. It aims to understand if staffing has got to do with the transmission of the deadly coronavirus. Thus, the study’s outcome could inform policy on staffing in nursing homes during the pandemic.

A simple Pearson test may be used to correlate one variable with another variable. But the study used multiple variables. Hence, they produced  regression models  that show how multiple variables affect the outcome. Some of the variables in the study may be redundant, meaning, those variables may represent the same attribute of a population.  Stepwise multiple regression models  take care of those redundancies. Using this statistical test requires further study and experience.

Topic 4: Surrounding Greenness, Stress, and Memory

Scientific evidence has shown that surrounding greenness has multiple health-related benefits. Health benefits include better cognitive functioning or better intellectual activity such as thinking, reasoning, or remembering things. These findings, however, are not well understood. A study, therefore, analyzed the relationship between surrounding greenness and memory performance, with stress as a mediating variable.

Statistical Research Question No. 4

Is there a significant relationship between exposure to and use of natural environments, stress, and memory performance?

As this article is behind a paywall and we cannot see the full article, we can content ourselves with the knowledge that three major variables were explored in this study. These are 1) exposure to and use of natural environments, 2) stress, and 3) memory performance.

Referring to the abstract of this study,  exposure to and use of natural environments  as a variable of the study may be measured in terms of the days spent by the respondent in green surroundings. That will be a ratio variable as we can count it and has an absolute zero point. Stress levels can be measured using standardized instruments like the  Perceived Stress Scale . The third variable, i.e., memory performance in terms of short-term, working memory, and overall memory may be measured using a variety of  memory assessment tools as described by Murray (2016) .

As you become more familiar and well-versed in identifying the variables you would like to investigate in your study, reading studies like this requires reading the method or methodology section. This section will tell you how the researchers measured the variables of their study. Knowing how those variables are quantified can help you design your research and formulate the appropriate statistical research questions.

Topic 5: Income and Happiness

This recent finding is an interesting read and is available online. Just click on the link I provide as the source below. The study sought to determine if income plays a role in people’s happiness across three age groups: young (18-30 years), middle (31-64 years), and old (65 or older). The literature review suggests that income has a positive effect on an individual’s sense of happiness. That’s because more money increases opportunities to fulfill dreams and buy more goods and services.

Reading the abstract, we can readily identify one of the variables used in the study, i.e., money. It’s easy to count that. But for happiness, that is a largely subjective matter. Happiness varies between individuals. So how did the researcher measured happiness? As previously mentioned, we need to see the methodology portion to find out why.

If you click on the link to the full text of the paper on pages 10 and 11, you will read that the researcher measured happiness using a 10-point scale. The scale was categorized into three namely, 1) unhappy, 2) happy, and 3) very happy.

Now the statistical test used by the researcher is, honestly, beyond me. I may be able to understand it how to use it but doing so requires further study. Although I have initially did some readings on logit models, ordered logit model and generalized ordered logit model are way beyond my self-study in statistics.

Anyhow, those variables found with asterisk (***, **, and **) on page 24 tell us that there are significant relationships between income and happiness. You just have to look at the probability values and refer to the bottom of the table for the level of significance of those relationships.

I do hope that upon reaching this part of the article, you are now well familiar on how to write statistical research questions. Practice makes perfect.

How is the quality of studies assessed.

How can you find out whether a study is any good? This question may sound odd at first because it is often assumed that every study provides new and usable findings. But unfortunately it isn’t that easy: A lot of studies do not provide reliable information.

So it is important to critically evaluate every study. This can be done in a systematic review that analyzes all the available studies on a specific medical issue.

In order to assess whether the results of a study are reliable, you first have to find out why the study was done in the first place and which questions it tried to answer. This may sound trivial, but it is crucial if you want to determine whether the study can actually provide any answers to the original research question. For instance, many studies compare a new medication with a placebo (a fake medication). But if there is already an effective treatment for the medical condition in question, the new medication is usually compared with that. After all, it is important for patients to know which treatment is most likely to work.

When assessing a study, the next step is to see whether the methods used are suitable for answering the research question, whether the study was carried out properly, and whether there were any systematic errors (bias) that could distort the results of the study.

  • Important questions to ask when assessing the quality of a study:
  • Is the study design suitable for answering the research question? For example, you can’t use a questionnaire to find out whether a new surgical procedure is better than a tried-and-tested approach. A randomized controlled trial (RCT) is needed instead.
  • How were the participants approached and selected? Who was included in the study and who was excluded from the study? For example, people who have several medical conditions at the same time are often excluded from studies. As a result, it might not be possible to apply the study outcomes to patients in the “real world” who have several medical conditions at the same time.
  • Was the researchers’ description of how they carried out the study detailed and understandable enough for others to be able to repeat the study and verify the results?
  • Were there enough participants in the study to be able to answer the research question? When treatments are compared, there are nearly always small differences between their outcomes. Scientists then work out the likelihood that these differences could be due to chance rather than being true differences. Here it is important to know exactly how different the outcomes were and how many people participated in the study: The smaller the difference, the more participants are needed in order to consider the difference to be “real.”
  • Are the endpoints that were used in the study suitable for demonstrating that patients benefit from the treatment? In a study on a diabetes medication, for instance, measuring blood sugar levels alone would not be enough. It would be more important to know whether the medication helps prevent long-term effects of diabetes such as amputations. Laboratory values like blood sugar levels (also referred to as “surrogate parameters”) alone are not enough to provide conclusive answers.
  • Did the study last long enough? To find out whether, for example, a certain weight loss diet is effective, the participants’ weight should be checked again six months or one year after the end of the study – perhaps even after a longer period of time.
  • How many participants dropped out of the study, and why? How many participants could no longer be monitored after the end of the main part of the study (“lost to follow-up”), and why not? Good studies should include these figures and say whether they influenced the outcomes. This may be the case, for instance, if a lot of people drop out of a study due to bad side effects.
  • Apart from receiving the different treatments that were being compared in the study, were the groups treated the same otherwise? Differences are especially likely if it wasn't possible to "blind" the doctors or participants properly.
  • Was it really a fair comparison? It could be a problem, for instance, if a new medication was compared with a standard medication used at a lower dose than usual in daily practice.
  • Was the success of treatment measured in the same way in both groups? For example, if the results of a blood test were used in one group, but both a blood test and an x-ray were used in the other group, that could change the outcome.
  • In order to assess the quality of RCTs, the following information is also needed:
  • How were the groups randomized? Were the participants really randomly assigned to the different groups, or did something influence their selection?
  • Where possible, did the researchers make sure that neither the study participants nor the doctors knew who was in which treatment group (blinding)?
  • Did all of the participants stay in the group they were originally assigned to throughout the study? If not, it is no longer possible to make a fair comparison between the groups at the end of the study.
