Cool Course: Disaster Nursing and Emergency Preparedness

Both historical and current events are the subjects of discussions and activities in Disaster Nursing and Emergency Preparedness, a course for upper-level undergraduate nursing students at NYU Rory Meyers College of Nursing.

Developed by Theresa Bucco , a clinical assistant professor at NYU Meyers, the course was first offered in 2019, but “once COVID hit, it became one of the most popular electives,” she says.

The course addresses the impact of a wide range of disasters—pandemics, natural disasters, mass casualty events, chemical and environmental disasters, and more—on individuals, communities, and the health care system. Students learn about the four phases of disasters (mitigation, preparedness, response, and recovery) through case studies, online FEMA trainings, guest speakers, and hands-on activities.

To teach it, Bucco draws on both personal experience—as an experienced critical care and emergency department nurse, which includes working on the frontlines of the pandemic—and the training she received through FEMA's Center for Domestic Preparedness in Anniston, Alabama.

During the semester, students learn how to use different types of protective equipment (PPE) in various situations, from gloves, gowns, and masks for infection control to self-contained breathing apparatuses for when air is unsafe to breathe.

Students also practice triaging or prioritizing cases in emergency rooms and on-site at disasters. In one exercise, students each receive a color-coded paper doll and are tasked with deciding who needs care most urgently depending on their injury or condition. In another class session, they receive a “Stop the Bleed” training that addresses how to respond to bleeding injuries and apply tourniquets.

Throughout the semester, students work in small groups to “dissect” a disaster. After choosing a historical incident to research, the group analyzes the event, the response by governments and organizations, the impact on the community, and the changes the disaster may have spurred.

“Disasters have led to so many different changes, from the Clean Air Act to new workplace safety regulations,” says Bucco. “For instance, the Cocoanut Grove fire, which was the deadliest nightclub fire in history, led to the development of specialty burn units in hospitals.”

These catastrophic events also take a psychological toll on health care workers, a topic the class discusses at length with a guest speaker, a nurse who has turned her focus to the wellbeing of other nurses since the pandemic.

Bucco notes that the ultimate goal of the course is to leave students feeling prepared for both the unexpected and the inevitable, whether they’re going on to roles in emergency departments, hospital emergency management, or other areas of health care.

“Most disaster experts agree that it's not a matter of if a disaster will occur, it's a matter of when ,” Bucco says.

Military organizations are often called upon to assist with emergency management missions, so it is critical that they have appropriately established and well understood organizational perceptual factors. Military organizations are often unique within a nation, which increases the challenge for scholars to effectively analyze how organizational factors are influenced by the dynamics of national use of military forces for emergency management missions. There are several disparate theories of government organization, but the most recent and relevant is Keith James' organizational science of disaster and terrorism prevention and response theory. James' theory identified several important organizational factors specific to emergency management organizations. These factors include organizational structure and networks, processes, teams, leadership, and technology. James' organizational factors guided the development of this qualitative comparative case study's interview protocol with 24 members from the strategic, operational, and tactical levels of the United States National Guard and Israeli Homefront Command. Results revealed why two nations use their military forces for disaster response as well as provided a description of how each organization is used within their respective nation. The data affirmed aspects of James' theory, including relevant structural, networking, and procedural factors and identified the other organizational factors within James' theory as possible areas for future research. Finally, based upon interview participant perceptions, recommendations were made to the leadership of the Homefront Command and National Guard regarding areas of potential organizational emphasis to include internal messaging, additional doctrine, and clarified organizational structures for disaster response.

  • The Great Unwinding And Its Effect On Skilled Nursing Facilities

Following the termination of the Public Health Emergency (“PHE”) as a result of COVID-19, and the continuous enrollment provisions put in place to ensure that Medicaid beneficiaries were able to receive Medicaid benefits through the PHE, states began to reimplement the renewal process – referred to by the Centers for Medicare & Medicaid Services (CMS) as an “unwinding”.

So far, since the start of the unwinding, KFF has estimated that approximately 17.7 million beneficiaries have been disenrolled from Medicaid/Chip programs as of March 4, 2024 since the start of the unwinding, which began in March 2023. This figure represents approximately 20% of persons enrolled in the Medicaid/Chip programs. According to KFF, there are almost 40 million renewals remaining to be processed.

States vary widely in the degree to which the unwinding has affected their Medicaid beneficiaries and the number of remaining beneficiaries who have yet to go through the renewal process. But what is clear across the board in looking at the renewal process is that many of the terminations are a result of “procedural reasons” versus an affirmative determination of ineligibility by the state Medicaid agency. According to KFF, 70% of those disenrolled from Medicaid/Chip were terminated for “procedural reasons”.

What are these “procedural reasons”? It is caused primarily by the beneficiaries (or legal agents) not receiving the renewal forms or not completing them timely. In our firm, we see this on a daily basis. It is not uncommon for persons to be terminated from Medicaid coverage because funds accumulated in a bank account during the pause in the renewal process resulted in them accumulating assets which now place them over the state’s resource limits (i.e., determined to be ineligible, not for a “procedural reason”). It is also not uncommon for persons to be terminated from coverage due to a failure of the beneficiary of their legal agent to respond at all to the Medicaid agency’s request for updated information about the beneficiary’s financial situation (i.e., terminated for a “procedural reason”).

This unwinding/renewal process, and the resulting disenrollments, have caused headaches and cash flow issues for skilled nursing facilities. There are ways that skilled nursing facilities can lessen the risk of future terminations of their residents, however. These include:

  • Talk to the resident or his/her legal agent or family members now about the renewal process. In many instances, family members are not familiar with this process. Warning the resident and family members about this process, and the need to respond to such timely (and risk of losing benefits) can go a long way to including them in the process and securing their cooperation in the renewal process.
  • Make sure to get updated contact information. A lot of people (understandably) may think that once approved for Medicaid benefits, there is nothing further that would need to be done in the coming years to ensure that eligibility is continued. The initial Medicaid eligibility process can oftentimes be so arduous, it is understandable that persons not familiar with the Medicaid process would not imagine there would be yearly requirements to ensure eligibility would continue for their loved one.
  • Talk to their resident and family members about the basic eligibility requirements and resource limits and remind them that the resource limit is an ongoing requirement and any resources over the limit must be spent down (in a Medicaid compliant manner) as soon as possible.

It was anticipated that the unwinding process would result in millions of people being disenrolled from Medicaid benefits and this has proven true. In many instances these are the very people who have no other payor source, and still require Medicaid benefits to meet their basic health needs, as much now as during the PHE. As we so often see, it is skilled nursing facilities, unfortunately, which bear the brunt of any fallout, here as a result of the unwinding. Taking some basic precautionary measures to respond to the post-PHE renewal process can do much to help the bottom line of skilled nursing facilities.
