We present a case of a 32-year-old male with a history of palpitations and preexcitation on ECG who underwent altogether four failed catheter ablations using different approaches in the two other electrophysiology centers within two years. ECG showed overt preexcitation with a positive delta wave in lead I and negative in leads V1 to V3, suggesting a right free wall accessory pathway. During the electrophysiological study, the accessory pathway was localized on the free lateral wall. However, the electrograms and mapping during atrial and ventricular pacing suggested the presence of true epicardial accessory pathway. Repeated radiofrequency energy delivery with the support of the steerable sheath and excellent contact (as assessed by intracardiac echocardiography) at the earliest ventricular activation was not successful. Therefore, the Farawave catheter (Boston Scientific, Inc) was used, and a flower configuration with the intention to cover the entire atrial attachment of the pathway during ventricular pacing was selected. Application of pulsed field resulted in interruption of accessory pathway conduction. An electrophysiological study one year later confirmed the persistent effect of ablation. This case illustrates the potential utility of pulsed field energy for the ablation of atrial insertion of the accessory pathway with an epicardial course. Such an approach can avoid epicardial mapping and access and may improve the safety of the procedure.

He just can’t stop losing body parts.

John Wayne Bobbitt — who made headlines in 1993 when his then-wife chopped off his penis while he slept — recently opened up about the most recent appendage he has lost: his toes.

Bobbitt, a former Marine, was diagnosed with toxic peripheral polyneuropathy after being exposed to contaminated water at the Camp Lejeune military training facility in the late 1980s.

The harrowing condition causes nerve damage to the extremities and caused Bobbitt to have all of his toes amputated over the years, the last of which were removed in 2023.

The operations left Bobbitt, 57, with two raw stumps for feet, a limp and needing special prosthetic shoes, he revealed in recent interview with the US Sun.

The condition can also lead to chronic infections , ulcers and painful skin grafts.

Bobbitt says his time at Camp Lejeune left him with more than just physical scars but mental ones that he believes contributed to his volatile relationship with his ex-wife, Lorena.

“Maybe I would have made better decisions if my cognitive functioning wasn’t distorted by the chemicals,” he told the outlet at his home in Sarasota, Florida.

Lorena Bobbitt claimed her then-husband raped and beat her and in June 1993, chopped off his manhood with a kitchen knife inside their Virginia apartment. She fled to her boss’ house and tossed the member out the window as she drove.

In a nine-hour surgery, doctors reattached Bobbitt’s salvaged penis and returned it to “normal” function , Bobbitt has sworn.

He went on to launch a porn career, and in 2013 claimed he slept with 70 women since the vengeful attack. That year, he nearly lost a foot after contracting an infection from a dirty nail at a construction site.

Federal prosecutors charged Ippei Mizuhara, longtime interpreter and close friend of Shohei Ohtani, with bank fraud Thursday, saying he stole more than $16 million from the Los Angeles Dodgers star to pay off gambling debts.

The indictment appeared to vindicate Ohtani in the scandal that has dominated headlines since the beginning of the baseball season, when the Los Angeles Times and ESPN broke the news that Ohtani’s name was on millions of dollars in wire transfers to a suspected illegal bookie, Mathew Bowyer .

In a news conference to announce the charges, U.S. Attorney Martin Estrada said that a thorough investigation had left no doubt that Mizuhara had “used and abused [his] position of trust to plunder Mr. Ohtani’s bank account to the tune of over $16 million.”

“I want to emphasize this point,” Estrada said. “Mr. Ohtani was a victim in this case.”

That would spell relief for Major League Baseball, for whom Ohtani — who has been both an elite slugger and pitcher throughout his career, earning comparisons to Babe Ruth — is its marquee player and a global icon. The Los Angeles Dodgers signed Ohtani , who is from Japan, to a record-shattering contract in December worth $700 million, the vast majority of it deferred.

Confusion was rampant as the initial news articles were published about the gambling payments, citing a tangle of conflicting explanations. An Ohtani spokesperson initially echoed Mizuhara’s account that the ballplayer had knowingly wired millions to cover his interpreter’s losses, according to ESPN. But the ballplayer’s attorneys soon disavowed that, saying that Ohtani was a victim of a “massive theft.”

MLB announced last month it was opening an investigation into the situation. After the indictment, the league issued a statement indicating it was putting its investigation on hold. “According to that investigation, Shohei Ohtani is considered a victim of fraud and there is no evidence that he authorized betting with an illegal bookmaker,” the MLB statement said. “Further, the investigation did not find any betting on baseball by Mr. Mizuhara. Given the information disclosed today, and other information we have already collected, we will wait until resolution of the criminal proceeding to determine whether further investigation is warranted.”

But the indictment Thursday also raised new questions about the scale of the wagering and the handling of the finances and public relations of one of the world’s best-known and highest-paid athletes.

Records cited in the complaint suggest far more massive gambling activity tied to Mizuhara than was previously reported — including wagers of more than $180 million. Overall, the account tied to the interpreter lost $41 million, according to prosecutors. Investigators stated that among those thousands of bets, there was no evidence that any of them were on baseball.

The indictment suggests that much of the confusion that allowed Mizuhara to allegedly siphon millions from Ohtani’s accounts and then use Ohtani’s own representatives to advance a false story exonerating him was the result of the interpreter using his control of a language barrier to manipulate not only the ballplayer but his agent, Nez Balelo, and a team of advisers and financial managers. Ohtani’s representatives declined to comment Thursday.

In interviews with federal investigators this month, according to the complaint, Ohtani denied authorizing the wire transfers and said he did not even learn about the impending scandal surrounding him until after Mizuhara and a spokesperson had already falsely told ESPN that Ohtani had knowingly paid off the debts. Ohtani said that following the season-opening game against the San Diego Padres in Seoul, Mizuhara addressed the entire Dodgers team in the clubhouse, in English, and Ohtani picked up that there was an issue. At a hotel later that evening after Ohtani confronted him, Mizuhara informed him for the first time that he had been paying his illegal bookie with funds from Ohtani’s account, the baseball star told the feds.

An IRS agent stated in a court affidavit that he had reviewed 9,700 pages of text messages between Ohtani and Mizuhara and found no discussion of betting wagers, debts or any other related matter. The agent, Chris Seymour, said he did “not find it credible” that Ohtani knew of Mizhura’s debts.

Mizuhara faces up to 30 years in prison if convicted. The New York Times reported earlier this week that Mizuhara was negotiating a guilty plea with federal prosecutors, but there was no indication of that in Thursday’s court filings. On Friday, when Mizuhara made his first appearance in court, his attorney, Michael Freedman, said in a statement that Mizuhara “is continuing to cooperate with the legal process and is hopeful that he can reach an agreement with the government to resolve this case as quickly as possible so that he can take responsibility.”

“He wishes to apologize to Mr. Ohtani, the Dodgers, Major League Baseball, and his family,” the statement added. “As noted in court, he is also eager to seek treatment for his gambling.”

Mizuhara’s relationship with Ohtani began near the end of 2017, after Ohtani signed with the Los Angeles Angels, his first MLB team. Ohtani didn’t speak English, and Mizuhara contacted him and offered his services as an interpreter. Yahoo has previously reported discrepancies in Mizuhara’s public résumé.

Mizuhara became inseparable from Ohtani, driving him and acting, according to the federal complaint, as his “de facto manager and assistant.” The complaint states that Ohtani’s agent, Balelo, didn’t hire any interpreters to deal with Ohtani because Mizuhara was always present. Baleo didn’t “speak directly” with his client or text him regularly, instead relaying messages through Mizuhara, according to the complaint. Ohtani’s financial team also did not include Japanese speakers, forcing them to rely on Mizuhara.

Mizuhara used that insulation and trust to prey on Ohtani, according to prosecutors. In March 2018, Mizuhara helped Ohtani set up a bank account in Arizona to deposit his playing checks. Mizuhara then kept the prying eyes of Ohtani’s financial advisers from the account, according to the indictment. When a bookkeeper attempted to gain access to it, Balelo informed him that, according to Mizuhara, Ohtani wanted the account “kept private.”

Another accountant told federal investigators that after he arranged a meeting with Ohtani, only Mizuhara showed, saying Ohtani was sick and that the ballplayer wanted the bank account in question “kept private from everyone.”

The account allegedly became a central fund for Mizuhara to pay off ballooning gambling debts. In November 2021, $40,010 was debited from the account in what an investigator called “the first unauthorized wire transfer” from the account by Mizuhara. Those transfers would reach well into the millions over the next couple of years as Mizuhara’s alleged wagering with an illegal bookie network spun out of control. Prosecutors said that bank recordings showed Mizuhara impersonated Ohtani, including answering security questions about his life, to trick bank employees into authorizing six-figure transfers to bookies.

The complaint does not name the bookie with whom Mizuhara primarily dealt, instead referring to him as “Bookmaker 1″ and saying he is a target of an ongoing investigation. But the details given, including that the bookie’s home was raided in October, align with those known about Bowyer. Bowyer declined to comment Thursday when reached by The Washington Post.

A log of gambling activity by the account linked to Mizuhara, which a federal investigator said was provided by a “cooperating source” in the illegal bookmaking network, suggested much larger dollar amounts than have been previously reported. The account linked to Mizuhara placed approximately 19,000 wagers in just over two years — or nearly 25 a day, ranging from $10 to $160,000 per bet, with an average bet of $12,600. The records reflect total winning bets of more than $142 million, according to the investigator, and total losing bets of nearly $183 million — meaning total losses of $40.7 million.

The complaint does not say whether other gamblers could have used the same account or otherwise explain the discrepancy between those losses and the $16 million cited by prosecutors.

Much of the most direct evidence tying Mizuhara to wagering came from his cellphone. The Dodgers fired Mizuhara in Korea, after Ohtani’s attorneys accused him of theft. Upon his return to the United States, according to the complaint, federal agents were waiting at Los Angeles International Airport and seized his phone.

A September 2021 text message between Mizuhara and the bookie suggested that the interpreter was a novice sports bettor. “I’ve just been messing around with soccer, there’s games on 24/7 lol,” Mizuhara texted, according to the complaint. “I took UCLA but they lost outright!!!”

But within eight months, the texts showed, Mizuhara had racked up more than $1 million in debt and appeared to beg for increasing amounts of credit — which the bookie seemed eager to extend to him.

“I’m terrible at this sport betting thing huh? Lol...” Mizuhara allegedly texted the bookie in November 2022. “Any chance u can bump me again?? As you know, you don’t have to worry about me not paying!!”

After a similar plea the following month, the bookie responded that giving him more credit was no problem, adding: “Merry Christmas.”

By the following June, the messages suggest, Mizuhara was wiring the bookie $500,000 a week while still asking for more credit. “I have a problem lol,” the interpreter allegedly wrote. “Can I get one last last last bump? This one is for real. . . . Last one for real[.]”

“Done,” the bookie responded, assuring him with a fist bump emoji. “I have the same problem. To be honest with you Ippie, as long as you can guarantee the 500 every Monday I’ll give you as much as you want because I know you’re good for it[.]”

But by last November, Mizuhara seemed to be out of the bookie’s good graces — and the bookie appeared to threaten outing him to Ohtani, who lived in Newport Beach, Calif. “Hey Ippie, it’s 2 o’clock on Friday,” reads a text message cited in the complaint. “I don’t know why you’re not returning my calls. I’m here in Newport Beach and I see [Ohtani] walking his dog. I’m just gonna go up and talk to him and ask how I can get in touch with you since you’re not responding? Please call me back immediately.”

Mizuhara allegedly responded a couple of days later, complaining about having lost money in cryptocurrency on top of the gambling losses. “Just wanted to ask, is there any way we can settle on an amount?” Mizuhara texted. “I’ve lost way too much on the site already . . . of course I know it’s my fault.”

The back-and-forth apparently came to a head around Jan. 6 , when the bookie texted Mizuhara that he was “putting me in a position where this is going to get out of control. If I don’t hear from you by the end of the day today it’s gonna [sic] be out of my hands.”

It’s not clear what Bowyer meant by that apparent threat. Bowyer’s attorney previously told The Post that she first spoke with federal authorities about Ohtani and Mizuhara that same month . But prosecutors say that as the walls closed in on Mizuhara, he continued to siphon from his famous client’s bank account. Prosecutors said that earlier this year, Mizuhara spent $325,000 on valuable baseball cards — including those of Yogi Berra, Juan Soto, and Ohtani himself — and had them mailed to him under a pseudonym at the Dodgers’ clubhouse.

The complaint states that Mizuhara and the bookie continued to text each other even as the news of the gambling debts broke. “Have you seen the reports?” Mizuhara asked after Ohtani’s attorneys had alleged the ballplayer was a victim of theft.

“Yes, but that’s all bulls---,” the bookie responded. “Obviously you didn’t steal from him. I understand it’s a cover job I totally get it.”

“Technically I did steal from him,” Mizuhara allegedly responded, adding: “it’s all over for me.”

Ohtani’s Former Interpreter Is Said to Be Negotiating a Guilty Plea

Ippei Mizuhara, the former translator for Shohei Ohtani who was fired late last month amid allegations he stole millions of dollars from the baseball star’s bank account to cover debts that Mizuhara owed to an illegal bookmaker, is in negotiations to plead guilty to federal crimes in connection with the purported theft, according to three people briefed on the matter.

The investigation, which began about three weeks ago after news of the alleged theft broke while Ohtani’s team, the Los Angeles Dodgers, was opening its season with two games in South Korea, is rapidly nearing a conclusion, according to the people, who spoke on the condition of anonymity because the inquiry is continuing.

A guilty plea from Mizuhara before a federal judge — likely to include an admission of a range of facts related to any illegal conduct — could confirm the account that Ohtani gave to reporters two weeks ago, in which he said he had no knowledge of what happened to the money.

Those briefed on the matter claim that prosecutors have uncovered evidence that Mizuhara may have stolen more money from Ohtani than the $4.5 million he was initially accused of pilfering, the people said. In particular, the authorities think they have evidence that Mizuhara was able to change the settings on Ohtani’s bank account so Ohtani would not receive alerts and confirmations about transactions, the three people said.

Ohtani’s lawyers initially alerted the federal authorities about the alleged theft, and Ohtani pledged publicly to cooperate with the federal investigation and one being conducted by Major League Baseball. According to one of the people briefed on the investigation, the federal authorities interviewed Ohtani in recent weeks to learn more about his relationship with Mizuhara.

By quickly pleading guilty, Mizuhara would increase his chances of receiving a more lenient sentence, as federal prosecutors and judges often look more favorably upon defendants who make the government’s job easier by expeditiously admitting their guilt.

Little is known about where Mizuhara has been since the Dodgers fired him. Upon returning to California from South Korea, Mizuhara was stopped by law enforcement officials after getting off the plane, the three people said. It’s unclear what Mizuhara told the authorities in their interaction, but he was not arrested.

Mizuhara has hired Michael Freedman, a former federal prosecutor in Los Angeles who specializes in white-collar criminal defense. Freedman declined to comment.

The investigation has been jointly led by the Los Angeles offices of the Internal Revenue Service’s criminal division and the Department of Homeland Security, along with the U.S. attorney’s office for the Central District of California.

A spokesman for the U.S. attorney’s office declined to comment. Matthew Hiltzik, a spokesman for Ohtani, referred to the player’s detailed explanation he gave to the media two weeks ago, when Ohtani said Mizuhara had stolen from him and he promised to cooperate fully with the federal and Major League Baseball investigations.

“I never bet on baseball or any other sports or never have asked somebody to do that on my behalf,” Ohtani said. “And I have never went through a bookmaker to bet on sports. Up until a couple days ago, I didn’t know this was happening.”

The allegations about the theft surfaced when the Dodgers were in Seoul to open the season with games against the San Diego Padres. Interest in the team has been intense since it signed Ohtani to a 10-year, $700 million contract in December. But as he and his new teammates were preparing for their opening games, reporters began asking about suspicious wire transfers from Ohtani’s account that had surfaced in a federal investigation of an alleged bookmaker. Mizuhara never informed Ohtani what was happening, Ohtani later told reporters.

Mizuhara, though, told Ohtani’s agent, Nez Balelo, that had discussed the matter with the player, according to Ohtani. But Mizuhara offered different versions to Balelo of what had occurred. First, Mizuhara said Ohtani had paid the debts of an unnamed teammate; then he said that he himself had racked up debts with the bookie and that Ohtani had bailed him out. The shifting stories alarmed executives in Major League Baseball, who worried that Ohtani might be tarnished by a connection to gambling.

Once executives with the Dodgers and Major League Baseball learned of the wire transfers — but with Ohtani still in the dark — the Dodgers asked Mizuhara to address the team in the clubhouse after the first game in Seoul. He told the team that he had a gambling addiction and was deep in debt, and that Ohtani, his close friend for years, had paid the debts.

At that point, Ohtani, who is not fluent in English but can understand the gist of some conversations, became suspicious. After Mizuhara’s clubhouse address, Ohtani told reporters, he confronted Mizuhara back at the team hotel. It was then, Ohtani said, that Mizuhara told him that he had stolen the money from his account. The Dodgers promptly fired him.

An earlier version of this article misstated what happened after reporters began asking about the wire transfers from Shohei Ohtani’s bank account. His interpreter, Ippei Mizuhara, told Ohtani’s agent, Nez Balelo, that he had discussed the matter with Ohtani, though Ohtani later said he had not. It is not the case that Balelo tried to manage the situation with Mizuhara without informing Ohtani.

Measles is a highly infectious febrile rash illness and was declared eliminated in the United States in 2000. However, measles importations continue to occur, and U.S. measles elimination status was threatened in 2019 as the result of two prolonged outbreaks among undervaccinated communities in New York and New York City. To assess U.S. measles elimination status after the 2019 outbreaks and to provide context to understand more recent increases in measles cases, CDC analyzed epidemiologic and laboratory surveillance data and the performance of the U.S. measles surveillance system after these outbreaks. During January 1, 2020–March 28, 2024, CDC was notified of 338 confirmed measles cases; 97 (29%) of these cases occurred during the first quarter of 2024, representing a more than seventeenfold increase over the mean number of cases reported during the first quarter of 2020–2023. Among the 338 reported cases, the median patient age was 3 years (range = 0–64 years); 309 (91%) patients were unvaccinated or had unknown vaccination status, and 336 case investigations included information on ≥80% of critical surveillance indicators. During 2020–2023, the longest transmission chain lasted 63 days. As of the end of 2023, because of the absence of sustained measles virus transmission for 12 consecutive months in the presence of a well-performing surveillance system, U.S. measles elimination status was maintained. Risk for widespread U.S. measles transmission remains low because of high population immunity. However, because of the increase in cases during the first quarter of 2024, additional activities are needed to increase U.S. routine measles, mumps, and rubella vaccination coverage, especially among close-knit and undervaccinated communities. These activities include encouraging vaccination before international travel and rapidly investigating suspected measles cases.

Introduction

Measles is a highly infectious acute, febrile rash illness with a >90% secondary attack rate among susceptible contacts ( 1 ). High national 2-dose coverage with the measles, mumps, and rubella (MMR) vaccine led to the declaration of U.S. measles elimination* in 2000 ( 2 ). However, this elimination status was threatened in 2019 because of two prolonged outbreaks among undervaccinated communities in New York and New York City; these outbreaks accounted for 29% of all reported cases during 2001–2019 ( 2 ). To assess U.S. measles elimination status after the 2019 outbreaks and to provide context for understanding more recent increases in measles cases in 2024, † CDC assessed the epidemiologic and laboratory-based surveillance of measles in the United States and the performance of the U.S. measles surveillance system during January 1, 2020–March 28, 2024.

Reporting and Classification of Measles Cases

Confirmed measles cases § ( 1 ) are reported to CDC by state health departments through the National Notifiable Disease Surveillance System and directly (by email or telephone) to the National Center for Immunization and Respiratory Diseases. Measles cases are classified by the Council of State and Territorial Epidemiologists as import-associated if they were internationally imported, epidemiologically linked to an imported case, or had viral genetic evidence of an imported measles genotype ( 1 ); cases with no epidemiologic or virologic link to an imported case are classified as having an unknown source ( 1 ). For this analysis, unique sequences were defined as those differing by at least one nucleotide in the N-450 sequence (the 450 nucleotides encoding the carboxyl-terminal 150 nucleoprotein amino acids) based on the standard World Health Organization (WHO) recommendations for describing sequence variants ¶ ( 3 ). Unvaccinated patients were classified as eligible for vaccination if they were not vaccinated according to Advisory Committee on Immunization Practices recommendations ( 4 ). A well-performing surveillance system was defined as one with ≥80% of cases meeting each of the following three criteria: classified as import-associated, reported with complete information on at least eight of 10 critical surveillance indicators (i.e., place of residence, sex, age, occurrence of fever and rash, date of rash onset, vaccination status, travel history, hospitalization, transmission setting, and whether the case was outbreak-related) ( 5 ), and laboratory-confirmed.

Assessment of Chains of Transmission

Cases were classified into chains of transmission on the basis of known epidemiologic linkages: isolated (single) cases, two-case chains (two epidemiologically linked cases), and outbreaks (three or more epidemiologically linked cases). The potential for missed cases within two-case chains and outbreaks was assessed by measuring the interval between measles rash onset dates in each chain; chains with more than one maximum incubation period (21 days) between cases could indicate a missing case in the chain. This activity was reviewed by CDC, deemed not research, and was conducted consistent with applicable federal law and CDC policy.**

Reported Measles Cases and Outbreaks

CDC was notified of 338 confirmed measles cases with rash onset during January 1, 2020–March 28, 2024 ( Figure ); cases occurred in 30 jurisdictions. During 2020, 12 of 13 cases preceded the commencement of COVID-19 mitigation efforts in March 2020. Among the 170 cases reported during 2021 and 2022, 133 (78%) were associated with distinct outbreaks: 47 (96%) of 49 cases in 2021 occurred among Afghan evacuees temporarily housed at U.S. military bases during Operation Allies Welcome, and 86 (71%) of 121 cases in 2022 were associated with an outbreak in central Ohio. During 2023, 28 (48%) of 58 cases were associated with four outbreaks. As of March 28, 2024, a total of 97 cases have been reported in 2024, representing 29% of all 338 measles cases reported during January 1, 2020–March 28, 2024, and more than a seventeenfold increase over the mean number of cases reported during the first quarter of 2020–2023 (five cases).

Characteristics of Reported Measles Cases

The median patient age was 3 years (range = 0–64 years); more than one half of cases (191; 58%) occurred in persons aged 16 months–19 years ( Table ). Overall, 309 (91%) patients were unvaccinated (68%) or had unknown vaccination status (23%); 29 (9%) had previously received ≥1 MMR vaccine dose. Among the 309 cases among unvaccinated persons or persons with unknown vaccination status, 259 (84%) patients were eligible for vaccination, 40 (13%) were aged 6–11 months and therefore not recommended for routine MMR vaccination, and 10 (3%) were ineligible for MMR because they were aged <6 months. †† Among 155 (46%) hospitalized measles patients, 109 (70%) cases occurred in persons aged <5 years; 142 (92%) hospitalized patients were unvaccinated or had unknown vaccination status. No measles-associated deaths were reported to CDC.

Imported Measles Cases

Among all 338 cases, 326 (96%) were associated with an importation; 12 (4%) had an unknown source. Among the 326 import-associated cases, 200 (61%) occurred among U.S. residents who were eligible for vaccination but who were unvaccinated or whose vaccination status was unknown. Among 93 (28%) measles cases that were directly imported from other countries, 34 (37%) occurred in foreign visitors, and 59 (63%) occurred in U.S. residents, 53 (90%) of whom were eligible for vaccination but were unvaccinated or whose vaccination status was unknown. One (2%) case in a U.S. resident occurred in a person too young for vaccination, two (3%) in persons who had previously received 1 MMR vaccine dose, and three (5%) in persons who had previously received 2 MMR vaccine doses. The most common source for internationally imported cases during the study period were the Eastern Mediterranean (48) and African (24) WHO regions. During the first quarter of 2024, a total of six internationally imported cases were reported from the European and South-East Asia WHO regions, representing a 50% increase over the mean number of importations from these regions during 2020–2023 (mean of two importations per year from each region).

Surveillance Quality Indicators

Overall, all but two of the 338 case investigations included information on ≥80% of the critical surveillance indicators; those two case investigations included information on 70% of critical surveillance indicators. Date of first case report to a health department was available for 219 (65%) case investigations; 127 (58%) cases were reported to health departments on or before the day of rash onset (IQR = 4 days before to 3 days after). Overall, 314 (93%) measles cases were laboratory confirmed, including 16 (5%) by immunoglobulin M (serologic) testing alone and 298 (95%) by real-time reverse transcription–polymerase chain reaction (rRT-PCR). Among 298 rRT-PCR–positive specimens, 221 (74%) were successfully genotyped: 177 (80%) were genotype B3, and 44 (20%) were genotype D8. Twenty-two distinct sequence identifiers (DSIds) ( 3 ) for genotype B3 and 13 DSIds for genotype D8 were detected (Supplementary Figure, https://stacks.cdc.gov/view/cdc/152776 ). The longest period of detection for any DSId was 15 weeks (DSId 8346).

Chains of Transmission

The 338 measles cases were categorized into 92 transmission chains (Table); 62 (67%) were isolated cases, 10 (11%) were two-case chains, and 20 (22%) were outbreaks of three or more cases. Seven (35%) of 20 outbreaks occurred during 2024. §§ The median outbreak size was six cases (range = three–86 cases) and median duration of transmission was 20 days (range = 6–63 days). Among the 30 two-case chains and outbreaks, more than one maximum incubation period (21 days) did not elapse between any two cases.

Because of the absence of endemic measles virus transmission for 12 consecutive months in the presence of a well-performing surveillance system, as of the end of 2023, measles elimination has been maintained in the United States. U.S. measles elimination reduces the number of cases, deaths, and costs that would occur if endemic measles transmission were reestablished. Investigation of almost all U.S. measles cases reported since January 2020 were import-associated, included complete information on critical surveillance variables, were laboratory-confirmed by rRT-PCR, and underwent genotyping; these findings indicate that the U.S. measles surveillance system is performing well. A variety of transmission chain sizes were detected, including isolated cases, suggesting that sustained measles transmission would be rapidly detected. However, the rapid increase in the number of reported measles cases during the first quarter of 2024 represents a renewed threat to elimination.

Most measles importations were cases among persons traveling to and from countries in the Eastern Mediterranean and African WHO regions; these regions experienced the highest reported measles incidence among all WHO regions during 2021–2022 ( 6 ). During November 2022–October 2023, the number of countries reporting large or disruptive outbreaks increased by 123%, from 22 to 49. Global estimates suggest that first-dose measles vaccination coverage had declined from 86% in 2019 to 83% in 2022, leaving almost 22 million children aged <1 year susceptible to measles ( 6 ).

As has been the case in previous postelimination years ( 7 ), most imported measles cases occurred among unvaccinated U.S. residents. Increasing global measles incidence and decreasing vaccination coverage will increase the risk for importations into U.S. communities, as has been observed during the first quarter of 2024, further supporting CDC’s recommendation for persons to receive MMR vaccine before international travel ( 4 ).

Maintaining high national and local MMR vaccination coverage remains central to sustaining measles elimination. Risk for widespread U.S. measles transmission remains low because of high population immunity; however, national 2-dose MMR vaccination coverage has remained below the Healthy People 2030 target of 95% (the estimated population-level immunity necessary to prevent sustained measles transmission) ( 8 ) for 3 consecutive years, leaving approximately 250,000 kindergarten children susceptible to measles each year ( 9 ). Furthermore, 2-dose MMR vaccination coverage estimates in 12 states and the District of Columbia were <90%, and during the 2022–23 school year, exemption rates among kindergarten children exceeded 5% in 10 states ( 9 ). Clusters of unvaccinated persons placed communities at risk for large outbreaks, as occurred during the central Ohio outbreak in 2022: 94% of measles patients were unvaccinated and 42% were hospitalized ( 10 ). Monitoring MMR vaccination coverage at county and zip code levels could help public health agencies identify undervaccinated communities for targeted interventions to improve vaccination coverage while preparing for possible measles outbreaks. As of March 28, 2024, a total of 97 confirmed measles cases have been reported in the United States in 2024, compared with a mean of five cases during the first quarter of each year during 2020–2023. Similar to cases reported during 2020–2023, most cases reported during 2024 occurred among patients aged <20 years who were unvaccinated or whose vaccination status was unknown, and were associated with an importation. Rapid detection of cases, prompt implementation of control measures, and maintenance of high national measles vaccination coverage, including improving coverage in undervaccinated populations, is essential to preventing measles and its complications and to maintaining U.S. elimination status.

Limitations

The findings in this report are subject to at least three limitations. First, importations might have been underreported: 4% of reported cases during the study period had no known source. Second, case investigations resulting in discarded measles cases (i.e., a diagnosis of measles excluded) are not nationally reportable, which limits the ability to directly evaluate the sensitivity of measles case investigations. However, surveillance remains sufficiently sensitive to detect isolated cases and outbreaks, and robust molecular epidemiology provides further evidence supporting the absence of sustained measles transmission in the United States. Finally, the date of first case report to a health department was not available for 35% of case investigations.

Implications for Public Health Practice

The U.S. measles elimination status will continue to be threatened by global increases in measles incidence and decreases in global, national, and local measles vaccination coverage. Because of high population immunity, the risk of widespread measles transmission in the United States remains low; however, efforts are needed to increase routine MMR vaccination coverage, encourage vaccination before international travel, identify communities at risk for measles transmission, and rapidly investigate suspected measles cases to maintain elimination.
