All cases must include a finalized corresponding report. For reporting criteria, refer to the Documentation Practice Parameter. OB Standard From the main site: 1 OB standard first-trimester complete transvaginal case with a live embryo or fetus; GA up to 14w. Transabdominal images may be submitted as adjunct only.

Limited or follow-up studies will not be accepted. Refer to page 2 for imaging checklist. Detailed 2nd Trimester OB Ultrasound From the main site: 2 (two) NORMAL indicated detailed second-trimester (76811) cases. (Non-indicated exams will not be accepted.) Refer to page 3 for imaging checklist. From a site of choice: 1 ABNORMAL indicated second-trimester detailed (76811) case with a major structural anomaly. (Non-indicated exams will not be accepted.) Refer to page 4 for additional instructions. Demonstrate the ONE TIME COMPONENTS (OTC). Refer to page 5 for additional instructions and imaging checklist. From each additional site: 1 NORMAL indicated detailed second-trimester (76811) case .

If you do not perform detailed 2nd trimester (76811) exams at the additional site, submit 1 complete OB s tandard second or third-trimester (76805) case , documenting all required anatomy. Limited or follow-up studies will not be accepted. 4 ALARA scans performed at less than 10 weeks GA should be done monitoring the Thermal Index for SOFT TISSUE (TIs) and scans performed at 10 weeks or greater GA should be done monitoring the Thermal Index for BONE (TIb). OB Standard 1st Trimester Labeled images of the following: 1. Uterus to determine location of gestational sac and rule out masses / abnormalities (at least 2 orthogonal planes) 2. Adnexal structures, even if ovaries not visualized 3. Cul-de-sac 4. Presence of embryo or fetus 5. Number of gestational sacs, embryos, or fetuses 6. Crown rump length 7. Demonstrate fetal heart rate using M-mode 8. If applicable, chorionicity and, if possible, amnionicity OB Standard 2nd/3rd Trimester (76805) Labeled images of the following: 1. Placental appearance and location in relation to internal os 2.
5 Placental cord insertion 3. Uterus and cervix 4. Adnexal structures, even if ovaries not visualized 5. BPD or HC measurement and landmarks 6. Abdominal circumference measurement and landmarks 7. 9 Defects that preclude a detailed examination of the entire fetus ( , Potter sequence with severe oligohydramnios, limb-body wall complex, anencephaly, etc.) will not be accepted, as the demonstration of the residual anatomy is generally not adequate for review. Anomalies acceptable for ABNORMAL 76811 Submission include but are not limited to the following list: Approval for Submission of an unlisted anomaly must be sought before Submission by emailing ABNORMAL Detailed 2nd Trimester (76811) The ABNORMAL 76811 exam: must be an indicated exam (non-indicated exams will not be accepted) must contain a major structural anomaly must demonstrate all images taken during a NORMAL detailed (76811) exam, PLUS any additional anatomy from the list below needed to make a diagnosis Accessory / succenturiate lobe with location of vascular supply to primary placenta Placental masses BPD or HC measurement and landmarks Abdominal circumference measurement and landmarks Femur length and landmarks Lateral cerebral ventricles Choroid plexus Midline falx 4th Ventricle 3rd Ventricle Cerebellar vermis Cerebellum Measurement of cerebellum Cisterna magna Cavum septi pellucidi Corpus callosum (sagittal view) Integrity and shape of cranial vault (can be shown in conjunction with other anatomy)
