Six Sigma organizations use Failure Modes and Effects Analysis (FMEA) to identify and improve processes, often before these failure modes cause defects and customer issues. All Fortune 500 companies, as well as many smaller companies, recognize FMEA’s as a leading tool to ensure they continually meet customer needs and requirements while avoiding costly defects and lost revenue.
FMEA’s include severity, occurrence/frequency, and detection methods for failure modes, making it easy to support process improvement projects. The risk priority numbers (RPN) provide valid results indicating those with the potential biggest impact, and the need to allocate Six Sigma resources appropriately to complete corrective actions. Here is a typical FMEA Map.
Develop a complete FMEA using the same topic for the Units 1, 2 and 3 lab assignments. Your FMEA must include the following:
